不同血压分级的原发性高血压患者血浆磷脂酶A2、CRP与颈动脉内膜中层厚度的相关性分析 被引量:1

摘要 目的分析不同血压分级的原发性高血压患者血浆磷脂酶A2(Lp-PLA2)、C反应蛋白(CRP)与颈动脉内膜中层厚度(IMT)的相关性。方法选取2018年1月至2019年5月苏州市吴江区中医医院收治的180例原发性高血压患者,以及同期接受的60例健康体检者作为研究对象。高血压患者根据血压水平,分为1级高血压(62例)、2级高血压(63例)和3级高血压(55例),检测各组患者的Lp-PLA2和CRP水平并进行比较。另根据颈动脉粥样硬化程度分为正常组(61例)、增厚组(62)和斑块形成组(57例),对各组患者的Lp-PLA2和CRP水平进行检测,比较各组患者的Lp-PLA2和CRP水平,采用Pearson相关性分析Lp-PLA2、CRP水平与IMT的相关性。结果高血压患者的Lp-PLA2和CRP水平高于健康体检者,差异有统计学意义(P<0.05);3级高血压患者的Lp-PLA2和CRP水平高于1级高血压和2级高血压,2级高血压患者的Lp-PLA2和CRP水平高于1级高血压,差异有统计学意义(P<0.05)。斑块形成组的Lp-PLA2和CRP水平高于正常组和增厚组,增厚组的Lp-PLA2和CRP水平高于正常组,差异有统计学意义(P>0.05)。采用Pearson相关性分析发现,血压分级与高血压患者Lp-PLA2、CRP水平呈正相关(r1=0.574,P1=0.008;r2=0.496,P=0.012),高血压患者Lp-PLA2、CRP与IMT呈正相关(r1=0.657,P1=0.000;r2=0.603,P=0.004)。结论不同血压分级的原发性高血压患者Lp-PLA2、CRP水平与IMT呈正相关,血压分级越高,Lp-PLA2、CRP水平,IMT越厚。 Objective To analyze the correlation between plasma phospholipase A2(Lp-PLA2),C reactive protein(CRP)and carotid intima-media thickness(IMT)in patients with essential hypertension with different blood pressure grades.Methods From January 2018 to May 2019,180 patients with essential hypertension and 60 healthy persons undergoing physical examination in our hospital were selected as the subjects.According to the level of blood pressure,patients with hypertension were divided into grade 1 hypertension(n=62),grade 2 hypertension(n=63)and grade 3 hypertension(n=55).The levels of Lp-PLA2 and CRP in each group were measured.The levels of Lp-PLA2 and CRP in each group were compared according to the level of Lp-PLA2 and CRP in the normal group(61 cases),the thickening group(62 cases)and the plaque formation group(57 cases)according to the degree of carotid atherosclerosis.The correlation between the levels of Lp-PLA2,CRP and IMT was analyzed by Pearson correlation.Results The levels of Lp-PLA2 and CRP in patients with high blood pressure were higher than those in healthy physical examination(P<0.05).The levels of Lp-PLA2 and CRP in patients with high blood pressure were higher than that of grade 1.The levels of Lp-PLA2 and CRP in patients with grade 2 hypertension and grade 2 hypertension were significantly higher than those in grade 1 hypertension(P<0.05).The levels of Lp-PLA2 and CRP in plaque formation group were higher than those in normal group and thickening group,and the levels of Lp-PLA2 and CRP in thickening group were significantly higher than those in normal group(P>0.05).Pearson correlation analysis showed that there was a positive correlation between blood pressure grade and Lp-PLA2,CRP level in patients with hypertension(r1=0.574,P1=0.008;r2=0.496,P=0.01).There was a positive correlation between Lp-PLA2,CRP and IMT in patients with hypertension(r1=0.657,P1=0.000;r2=0.603,P=0.004).Conclusion There was a positive correlation between Lp-PLA2,CRP and IMT in essential hypertension patients with different blood pressure grades.the higher the blood pressure grade,the thicker the Lp-PLA2,CRP and IMT.
