丹红注射液联合依达拉奉注射液治疗进展性缺血性脑卒中的疗效观察 被引量:5

Observation on Efficacy of Danhong Injection Combined with Edaravone Injection in Treatment of Progressive Ischemic Stroke

摘要 目的:探讨丹红注射液联合依达拉奉注射液治疗进展性缺血性脑卒中的疗效。方法:采用回顾性分析的方式,将2015年7月至2016年8月十堰市太和医院收治的63例进展性缺血性脑卒中患者设为对照组,将2016年9月至2017年9月收治的64例进展性缺血性脑卒中患者设为观察组。两组患者均接受脑卒中单元个体化治疗2周,观察组患者在对照组的基础上加用丹红注射液+依达拉奉注射液静脉滴注。监测两组患者局部脑血流(RCBF),比较两组患者血液流变学指标、血脂代谢指标、血清可溶性细胞间黏附分子-1(sICAM-1)和血管细胞黏附分子-1(VCAM-1)水平的差异,观察两组患者日常生活能力(activity of daily living,ADL)评分、美国国立卫生院卒中神经功能缺损评分量表(national institutes of health stroke scale,NIHSS)评分及临床疗效。结果:治疗1、2周,观察组患者的RCBF明显高于对照组,差异均有统计学意义(P<0.05);治疗后,观察组患者的纤维蛋白原、血小板聚集率、全血高切黏度、全血低切黏度和红细胞比容明显低于对照组,差异均有统计学意义(P<0.05);治疗后,观察组患者的三酰甘油、总胆固醇及低密度脂蛋白胆固醇水平明显低于对照组,而高密度脂蛋白胆固醇水平明显高于对照组,差异均有统计学意义(P<0.05);治疗后,观察组患者sICAM-1和VCAM-1水平明显低于对照组,差异均有统计学意义(P<0.05);治疗2周和随访3个月时,观察组患者ADL评分明显高于对照组,NIHSS评分明显低于对照组,差异均有统计学意义(P<0.05);观察组患者的总有效率为75.00%(48/64),明显高于对照组的65.08%(41/63),差异有统计学意义(P<0.05)。结论:丹红注射液联合依达拉奉注射液治疗进展性缺血性脑卒中的疗效显著,有利于降低患者血液黏性和血脂水平,增加RCBF,改善微循环,并促进神经功能恢复。 OBJECTIVE: To probe into the efficacy of Danhong injection combined with edaravone injection in treatment of progressive ischemic stroke(PIS). METHODS: 63 patients with PIS admitted into Taihe Hospital of Shiyan city from Jul. 2015 to Aug. 2016 were retrospectively selected and set as control group, another 64 patients with PIS admitted from Sept. 2016 to Sept. 2017 were set as observation group. Both groups were received 2 weeks of individualized treatment during stroke unit, the observation group was given Danhong injection combined with edaravone injection for intravenous drip based on the control group. The part cerebral blood flow(RCBF) of both groups were monitored, differences in indices of hemorheology and blood-lipid metabolism, levels of serum solubility intercellular adhesion molecule-1(sICAM-1) and vascular cell adhesion molecule-1(VCAM-1) between two groups were compared. Scores of activity of daily living(ADL), national institutes of health stroke scale(NIHSS) and clinical efficacy of the two groups were observed. RESULTS: At the first and second weeks of treatment, the RCBF level of observation group was significantly higher than that of the control group, with statistically significant difference(P<0.05);after treatment, the fibrinogen, platelet aggregation rate, high shear viscosity of whole blood, low shear viscosity of whole blood and hematokrit of observation group were significantly lower than those of the control group, with statistically significant differences(P<0.05);after treatment, levels of triacylglycerol, total cholesterol and low density lipoprotein cholesterin of observation group were significantly lower than those of the control group, while the high density lipoprotein cholesterin of observation group were significantly higher, with statistically significant differences(P<0.05);after treatment, the sICAM-1 and VCAM-1 levels of observation group were significantly lower than those of the control group, with statistically significant differences(P<0.05);at 2 weeks of treatment and 3 months of follow-up, the ADL scores of observation group were significantly higher than those of the control group, while the NIHSS scores of observation group were significantly lower, with statistically significant differences(P<0.05);the total effective rate of observation group was 75.00%(48/64), which was significantly higher than that of the control group(65.08%, 41/63), with statistically significant difference(P<0.05). CONCLUSIONS: The efficacy of Danhong injection combined with edaravone injection in treatment of PIS is remarkable, which is beneficial to reduce blood viscosity and blood lipid, increase RCBF, improve microcirculation and promote the recovery of neurological function.
