Investigation and Analysis of Oral Unit Dose Dispensing System in Hospitalized Children Under JCI Criteria

摘要 目的:了解昆明医科大学附属儿童医院(以下简称"我院")住院患儿口服单剂量药物使用情况,为规范单剂量的操作,改善病区药品管理质量,保障患儿用药安全、经济提供参考。方法:通过信息系统,提取2018年我院住院患儿口服单剂量药物的使用资料,根据美国医疗机构国际联合委员会(joint commission international,JCI)标准和药物经济学指南,确定用量最多的药物类别及其代表药品,进行调查与分析。结果:呼吸系统用药、消化系统用药和肠内肠外营养与调节水、电解质及酸碱平衡药是我院最常拆分的3大类药物,其代表药品包括氨溴特罗口服液、酪酸梭菌二联活菌散、蒙脱石散和赖氨葡锌颗粒;氨溴特罗口服液2.5、5和10 ml,蒙脱石散1、1.5和3 g,赖氨葡锌颗粒125 mg、5 g和250 mg等规格用量较大。结论:儿童消化、呼吸系统用药规格短缺,建议拆分单剂量或药品生产企业增加药品规格。单剂量发药为住院患儿节约了一定的医药费用,提高了药学服务质量,增加了医院的社会效益。 OBJECTIVE: To investigate the application of oral unit dose dispensing system in hospitalized children in Children’s Hospital of Kunming Medical University(hereinafter referred to as "our hospital"), so as to provide reference for standardizing unit dose dispensing system, improving drug management quality in the ward, and ensuring medication safety and economy for children. MRTHODS: Through the information system, the data on the application of oral unit dose dispensing system in hospitalized children from our hospital in 2018 were extracted. According to the Joint Commission International(JCI) standards and the Guidelines For Pharmacoeconomics, the most widely used drug categories and the representative drugs were investigated and analyzed. RESULTS: Respiratory system drugs, digestive system drugs and enteral parenteral nutrition and regulating water, electrolytes and acid-base balance drugs were the three most commonly dissected drugs in our hospital, and the representative drugs were respectively ambrobuterol oral solution, clostridium tyrosinate didiviable powder, montmorillonite powder and Lyamine zinc granules. There were large amount of specifications, including ambroxol oral solution 2.5, 5 and 10 ml, montmorillonite powder 1, 1.5 and 3 g, lysine zinc granules 125 mg, 5 g and 250 mg, etc. CONCLUSIONS: There is a shortage of specifications for children’s digestive and respiratory systems. It is recommended to use unit dose dispensing system or increase drug specifications by pharmaceutical manufacturers. Unit dose dispensing system saves some medical expenses for hospitalized children, improves the quality of pharmaceutical services, and increases the social benefits of the hospital.
