药师干预对社区老年慢性病患者用药的影响

Effects of Pharmacists’ Intervention on Medication of Elderly Patients with Chronic Diseases in the community

目的:探讨药师个体化干预对社区老年慢性病患者用药的影响。方法:对2018年3-10月纳入的100例社区老年慢性病患者用药实行药师个体化干预,分析干预前后患者在用药数量、服药依从性、接受用药指导程度、不良反应发生情况及药费支出情况等方面的变化,评估干预效果。结果:通过药师的个体化干预,患者的用药依从性、部分不良反应发生情况及接受用药指导程度均得到了显著改善,差异均有统计学意义(P<0.01);而干预前后患者用药数量、药费支出情况的差异均无统计学意义(P>0.05)。结论:药师对社区老年慢性病患者用药进行个体化干预,可以显著提高患者用药的依从性、有效性,减少不良反应的发生,促进患者合理用药,有利于提高患者的健康状况和生活质量。

OBJECTIVE: To investigate the effects of pharmacists’ intervention on medication of elderly patients with chronic diseases in the community. METHODS: Totally 100 cases of community elderly patients with chronic diseases were included in the individualized pharmacists’ intervention on medication from Mar. to Oct. 2018. Changes of the patients’ medication quantity, medication compliance, degree of receiving medication guidance, adverse drug reactions and drug expenditure before and after intervention were analyzed, and the intervention effects were evaluated. RESULTS: Through the individualized intervention by pharmacists, medication compliance, incidence of some adverse drug reactions, and acceptance of medication guidance of patients were significantly improved, with statistically significant differences(P<0.01). There was no significant difference in the medication quantity and drug expenditure before and after intervention(P>0.05). CONCLUSIONS: Pharmacists’ individualized intervention on medication for elderly patients with chronic diseases in the community can significantly improve the compliance and effectiveness of medication for patients, reduce the incidence of adverse drug reactions, promote the rational drug use for patients, and improve the health status and quality of life.
