摘要
目的研制全脊椎切除术椎间盘专用拉钩。方法通过磁化原理设计的全脊椎椎间盘专业拉钩由拉钩体、拉钩柄、磁化座构成。将该全脊椎切除术椎间盘专用拉钩应用于2例椎体转移瘤行全脊椎切除重建术的患者并评估临床使用效果。结果术中先完成椎弓根螺钉固定,再切除椎板、椎弓根,随后在切除椎间盘时,将两个椎间盘拉钩合二为一,呈270°环抱椎体。再加以磁化,引导穿入线锯顺利切开椎间盘,整块椎体即被完整切除,最后完成椎体重建。整个手术术野显露良好,无损伤血管、神经的并发症发生。结论采用磁化技术的全脊椎切除术椎间盘专用拉钩可以充分显露手术术野,具有使用安全、可靠的特点。
Objective To develop a special retractor for intervertebral disc for total spinal resection.Methods The professional spine of the full-vertebral disc designed by the principle of magnetization is composed of a hook body,a hook handle and a magnetized seat.We applied this total spinal resection disc hook to patients who underwent total spinal resection and reconstruction of 2 cases of vertebral metastases and evaluated the clinical use.Results The pedicle screw fixation was performed first,then the lamina and pedicle were removed.Then,when the intervertebral disc was removed,the two intervertebral discs were combined into one,which was 270°surrounded by the vertebral body.Then magnetized,guided through the wire saw to cut the intervertebral disc smoothly,the whole vertebral body was completely removed,and finally the vertebral body reconstruction.The entire surgical field was well exposed,and no complications such as damage to blood vessels and nerves occurred.Conclusion The special ligation of the intervertebral disc with magnetization technology can fully reveal the surgical field and it is safe and reliable.
