牙周牙髓联合半导体激光治疗重度牙周炎的临床疗效 被引量:20

Clinical Study of Periodontal Endodontic Treatment Combined with Semiconductor Laser Treatment for Severe Periodontitis

摘要 目的探讨牙周牙髓联合半导体激光治疗重度牙周炎的临床疗效。方法收集2017年3月至2018年3月在合肥市第二人民医院口腔科治疗的重度牙周炎患者50例(50颗患齿)为研究对象,根据治疗方式不同分为两组,对照组(n=24)采用牙周牙髓治疗,研究组(n=26)采用牙周牙髓联合半导体激光治疗,记录两组治疗前和治疗后3个月、6个月、12个月探诊深度(PD)、附着丧失(AL)水平、出血指数(BI),治疗前和治疗后12个月牙齿松动度,并观察两组治疗前后根尖片。结果治疗前两组PD差异无统计学意义(P>0.05),治疗6个月、12个月PD均低于治疗前(P<0.05),治疗12个月研究组PD小于对照组[(5.60±0.21)mm比(6.35±0.38)mm](P<0.05);治疗前两组AL比较差异无统计学意义(P>0.05),治疗3个月、6个月、12个月两组AL均小于治疗前(P<0.05),治疗12个月研究组AL低于对照组[(5.79±0.22)mm比(6.16±0.26)mm](P<0.05);治疗前、治疗3个月两组BI比较差异无统计学意义(P>0.05),治疗6个月研究组BI低于对照组[(0.62±0.11)比(1.95±0.77)](P<0.05);治疗12个月研究组BI低于对照组[(0.45±0.12)比(1.76±0.76)](P<0.05)。治疗前后两组牙齿松动度比较差异无统计学意义(P>0.05);治疗12个月后,研究组、对照组均有明显再生的牙槽骨。结论牙周牙髓治疗联合半导体激光治疗较单纯牙周牙髓治疗能更有效地改善重度牙周炎患者牙周组织。 Objective To discuss the clinical efficacy of periodontal endodontic treatment combined with semiconductor laser treatment for severe periodontitis.Methods A total of patients(50 affected teeth)with severe periodontitis treated in Hefei Second People's Hospital from Mar.2017 to Mar.2018 were included.According to the treatment method,they were divided into a control group(n=24)given periodontal endodontic treatment,and a study group(n=26)given eriodontal endodontic treatment combined with semiconductor laser treatment.Probing depth(PD),attachment loss(AL)level,bleeding index(BI)before treatment and 3,6,12 months after treatment,and tooth mobility before treatment and 12 months after treatment were recorded,and the apical slices before and after treatment were observed.Results Before treatment,there was no significant difference in PD between the two groups(P<0.05).The PD at 6 months and 12 months was lower than that before treatment(P<0.05).After 12 months of treatment,the study group's PD was smaller than the control group's PD[(5.60±0.21)mm vs(6.35±0.38)mm](P<0.05).Before treatment,the AL difference between the two groups was not statistically significant(P>0.05),at 3 months,6 months and 12 months after treatment AL of both groups were less than before treatment(P<0.05),after 12 months of treatment,the study group's AL was less than that of the control group[(5.79±0.22)mm vs(6.16±0.26)mm](P<0.05);before treatment,after 3 months of treatment,there was no significant difference in BI between the two groups(P>0.05),after 6 months of treatment,the study group's BI was lower than the control group's[(0.62±0.11)vs(1.95±0.77)](P<0.05);after 12 months of treatment the study group's BI was lower than the control group's[(0.45±0.12)vs(1.76±0.76)](P<0.05).There was no significant difference in tooth mobility between the two groups before and after treatment(P>0.05).After 12 months of treatment,there were significantly regenerated alveolar bone in both groups.Conclusion Periodontal endodontic treatment combined with semiconductor laser treatment can improve the periodontal tissue of patients with severe periodontitis more effectively than periodontal endodontic treatment alone.
