阴腹联合超声在附件包块患者术前诊断中的应用

Application of combined pudendal and abdominal ultrasonography in preoperative diagnosis of adnexal mass

目的:探讨阴腹联合超声在附件包块患者术前诊断中的应用。方法:回顾性分析2016年6月到2019年4月经手术病理确诊的660例附件包块患者的临床资料,患者术前均经阴道超声和腹部超声进行诊断。比较阴腹联合超声与阴道超声对附件包块性质诊断准确率、诊断效能。结果:660例附件包块患者中,良性包块528例,恶性包块132例。阴腹联合超声的诊断准确率、良性包块诊断准确率和恶性包块诊断准确率分别为97.27%、95.45和98.48%,显著高于阴道超声的80.00%、83.33%和66.67%,差异具有统计学意义(P<0.05)。阴腹联合超声对附件包块性质的灵敏度和特异性分别为84.42%和99.60%,显著高于阴道超声的50.00%和90.91%,差异具有统计学意义(P<0.05)。结论:阴腹联合超声对附件包块性质区分诊断准确性较高,值得广泛推广。

Objective To investigate the application of combined pudendal and abdominal ultrasonography in preoperative diagnosis of adnexal mass.Methods The clinical data of 660 cases of adnexal mass confirmed by operation and pathology from June 2016 to April 2019 were retrospectively analyzed.All patients were diagnosed by transvaginal ultrasound and abdominal ultrasound before operation.To compare the diagnostic accuracy and efficiency of combined vaginal and abdominal ultrasonography in the diagnosis of adnexal mass.Results Among 660 cases of adnexal mass,528 cases were benign mass and 132 cases were malignant mass.The diagnostic accuracy of combined transvaginal and abdominal ultrasound,benign mass and malignant mass were 97.27%,95.45 and 98.48%respectively,which were significantly higher than that of transvaginal ultrasound(80.00%,83.33%and 66.67%,P<0.05).The sensitivity and specificity of combined transvaginal and abdominal ultrasound for the nature of adnexal mass were 84.42%and 99.60%,respectively,which were significantly higher than those of transvaginal ultrasound(50.00%and 90.91%,P<0.05).Conclusion Ultrasound combined with pudenda and abdomen has high diagnostic accuracy in distinguishing the nature of adnexal mass,and it is worth popularizing widely.
