无创呼吸机联合治疗冠心病合并OSAHS的疗效观察

摘要 目的观察常规药物联合无创呼吸机治疗冠心病合并睡眠呼吸暂停综合征(OSAHS)的疗效。方法选择2018年1月-2019年1月医院门诊治疗的62例冠心病合并OSAHS患者,按随机数字表法分为两组,均31例。对照组接受常规药物治疗,观察组则加用无创呼吸机治疗。观察两组夜间最低血氧饱和度(SaO2)、睡眠呼吸暂停低通气指数(AHI)、血液流变学指标[全血低切黏度(LBV)、全血高切黏度(HBV)]及冠心病症状评分。结果治疗前两组AHI、夜间SaO2、LBV、HBV对比,差异无统计学意义(P> 0.05);治疗后观察组AHI(6.31±2.15)次/h、LBV(15.20±1.41)mPa/s、HBV(3.01±0.34)mPa/s、胸痛(2.08±0.79)分、胸闷(2.23±0.83)分、气短(1.72±0.86)分症状评分低于对照组,夜间SaO2(94.65±2.61)%高于对照组,差异有统计学意义(P <0.05)。结论冠心病合并OSAHS治疗中选取常规药物、无创呼吸机联合治疗有助于缓解患者症状,改善血液流变学指标、夜间缺氧状态。 Objective To observe the efficacy of conventional drugs combined with non-invasive ventilator in the treatment of coronary heart disease combined with sleep apnea syndrome(OSAHS). Methods From January 2018 to January 2019, 62 patients with coronary heart disease and OSAHS treated in our hospital were selected and divided into two groups according to the random number table method, with 31 cases each. The control group received conventional drug treatment, and the observation group was treated with non-invasive ventilator. Observe the minimum blood oxygen saturation(SaO2), sleep apnea hypopnea index(AHI), and hemorheology index [low blood viscosity(LBV), high blood viscosity(HBV)]. Results There was no significant difference in AHI, night SaO2,LBV and HBV between the two groups before treatment(P > 0.05);AHI(6.31±2.15) times/h, LBV(15.20±1.41) MPA/s, HBV(3.01±0.34) MPA/s,chest pain(2.08±0.79), chest distress(2.23±0.83) and shortness of breath(1.72±0.86) in the observation group after treatment were lower than those in the control group, while SaO2(94.65±2.61)% at night was higher than those in the control group The difference was statistically significant(P< 0.05). Conclusion In the treatment of coronary heart disease combined with OSAHS, the use of conventional drugs and non-invasive ventilator combined therapy can help control the disease progression, improve blood rheology indicators, and nocturnal hypoxia.
