摘要
目的探讨血清糖类抗原125(CA125)与慢性阻塞性肺疾病(COPD)患者肺动脉压力的关系及意义.方法选取2016年1月至2018年1月在某院治疗的COPD患者134例,其中COPD合并肺动脉高压(PAH)患者43例(COPD合并PAH组),单纯COPD患者91例(单纯COPD组),同时选取健康志愿者100例作为对照组,采用彩色多普勒心脏超声评估肺动脉收缩压(PASP),检测各组血清CA125、癌胚抗原(CEA)及细胞角蛋白片段(CYFRA).结果COPD合并PAH组PASP为(55.82±6.72)mmHg(1 mmHg=0.133 kPa),高于单纯COPD组和对照组(P<0.05);单纯COPD组PASP为(27.83±3.11)mmHg,高于对照组(P<0.05);COPD合并PAH组CA125为(36.92±5.88)U/mL,明显高于单纯COPD组和对照组(P<0.05);单纯COPD组CA125为(29.02±6.10)U/mL,明显高于对照组(P<0.05);各组CEA和CYFRA比较差异无统计学意义(P>0.05);COPD患者血清CA125与PASP呈正相关(r=0.533,P<0.05);CA125判断COPD合并PAH的ROC曲线下面积为0.850,P<0.05,截断值为32.31 U/mL,灵敏度为83.70%,特异性为73.60%.结论血清CA125与COPD患者肺动脉压力有相关性,在判断COPD合并PAH中有一定应用价值.
Objective To investigate the relationship between serum carbohydrate antigen 125(CA125)and pulmonary arterial pressure in patients with chronic obstructive pulmonary disease(COPD)and its significance.Methods 134 COPD patients treated in a hospital from January 2016 to January 2018 were selected of which 43 cases were diagnosed with COPD combined with pulmonary hypertension(PAH)(COPD combined with PAH)and 91 cases of COPD(COPD group).100 healthy volunteers were selected as the control group.Pulmonary systolic pressure(PASP)was evaluated by color Doppler echocardiography and serum CA125,carcinoembryonic antigen(CEA)and Cytokeratin fragment(CYFRA)were detected in each group.Results The PASP of COPD combined with PAH group was(55.82±6.72)mmHg,higher than that in the COPD group and the control group(P<0.05).The PASP in the COPD group was(27.83±3.11)mmHg,higher than that in the control group(P<0.05).The CA125 of COPD combined with PAH group was(36.92±5.88)U/mL,which was significantly higher than that in the COPD group and the control group(P<0.05).The CA125 in the COPD group was(29.02±6.10)U/mL,which was significantly higher than that in the control group(P<0.05).There were no significant difference in CEA and CYFRA between each groups(>0.05).There was a positive correlation between serum CA125 and PASP in COPD patients(=0.533,<0.05).The area under the ROC curve of the COPD combined with PAH judged by CA125 was 0.850,with<0.05.The cut-off value was 32.31 U/mL,the sensitivity was 83.70%and the specificity was 73.60%.Conclusion There was a correlation between serum CA125 and pulmonary artery pressure in COPD patients,enjoying certain application value in the diagnosis of COPD combined with PAH.
