观察急性重症左心衰竭患者早期辅助无创正压通气的疗效

To observe the effect of early assisted noninvasive positive pressure ventilation in patients with acute severe left heart failure

目的观察急性重症左心衰竭患者早期辅助无创正压通气治疗的临床效果。方法选取2018年4月至2019年1月在本院治疗的急性重症左心衰竭患者102例,采用随机方法分为对照组和观察组,各51例。对照组患者给予常规对症支持治疗,观察组患者在对照组的基础上给予早期辅助无创正压通气治疗,观察并比较两组患者的临床治疗效果以及治疗前后的呼吸(rspiration,R)、心率(heart rate,HR)、氧/二氧化碳分压(PaO2/PaCO2)、血氧饱和度SaO2)、血浆钠尿肽(B type natriuretic peptide,BNP)等水平的变化情况。结果治疗前,两组患者的R、HR、PaO2/PaCO2、SaO2、BNP等水平比较差异均无统计学意义;但治疗后,两组患者的各项指标均有所改善,观察组PaO2、SaO2高于对照组,其余指标低于对照组,差异有统计学意义(P<0.05);观察组的治疗总有效率(96.08%)高于对照组(68.63%),差异有统计学意义(P<0.05)。结论临床上治疗急性重症左心衰竭患者时,应给予其早期辅助无创正压通气治疗,能够有效改善患者的血气分析指标,减轻患者心力衰竭的症状,效果显著,有较高的临床推广价值。

Objective To observe the clinical effect of early assisted noninvasive positive pressure ventilation in patients with acute severe left heart failure.Methods 102 patients with acute severe left heart failure in our hospital from April 2018 to January 2019 were randomLy divided into control group and observation group,with 51 cases in each group.Patients in the control group were given routine symptomatic support treatment,while patients in the observation group were given early non-invasive positive pressure ventilation treatment on the basis of the control group.The clinical efficacy,respiratory(R),heart rate(HR),PaO2/PaCO2 before and after treatment were observed and compared between the two groups.The changes of PaO2/PaCO2,pulse oxygen saturation(SaO2)and plasma natriuretic peptide(BNP)levels were observed.Results Before treatment,there were no significant differences in R,HR,PaO2/PaCO2,SaO2 and BNP between the two groups.But after treatment,the indexes of the two groups were improved.PaO2 and SaO2 in the observation group were higher than those in the control group,and the other indexes were lower than those in the control group(P<0.05).The total effective rate of the observation group(96.08%)was higher than that in the control group(68.63%).The difference was significant(P<0.05).Conclusion Early non-invasive positive pressure ventilation should be given to patients with acute severe left heart failure,which can effectively improve the blood gas analysis indicators of patients and alleviate the symptoms of heart failure.The effect is remarkable and has a high popularization value.
