动态血压监测联合胸阻抗血流动力学监测在顽固性高血压诊疗中的应用

Dynamic blood pressure monitoring combined with chest impedance hemodynamic monitoring in patients with refractory hypertension

目的观察动态血压监测(ABPM)联合胸阻抗血流动力学监测(ICG)在顽固性高血压患者中的应用效果。方法收入2016年6月至2019年6月收治的顽固性高血压患者60例,入组后随机分为观察组与对照组,观察组30例治疗前给予动态血压及血流动力学监测;同期另30例患者为对照组,治疗前给予动态血压监测;临床医生根据监测结果参数进行综合分析并给予针对性的个性化用药及护理。1个月后患者回院随访进行动态血压监测以观察评价患者高血压治疗效果。结果观察组总有效率更高,降压效果显著,与对照组对比差异有统计学意义(P<0.05)。结论动态血压监测联合胸阻抗血流动力学监测应用在顽固性高血压患者诊疗中,为临床医生提供了可靠的诊断治疗依据,结合患者的实际情况选择合适的降压药物和服药时间,进行针对性、个体化治疗干预,降压效果显著,具有重要临床价值。

To evaluate effects of simple ambulatory blood pressure monitor and combined ambulatory blood pressure with chest impedance hemodynamic monitoring in treating patients with intractable hypertension.Methods 60 patients with intractable hypertension,admission to hospital from June,2016 to June,2019,were randomly divided into control group(simply ambulatory monitor)and combination group,each group consists 30 patients.Clinicians carry out comprehensive analysis according to the parameters of monitoring results and give targeted,individualized drug use and nursing care.After one month,the patients were followed up for ambulatory blood pressure monitoring to observe and evaluate the therapeutic effect of hypertension.Results The overall efficiency was higher and the antihypertensive effect was significant in the combined monitoring group than control group(P<0.05).Conclusion Ambulatory blood pressure monitoring combined with chest impedance hemodynamic monitoring provides a reliable basis for clinicians to diagnose and treat intractable hypertension.According to the actual situation of patients,appropriate antihypertensive drugs and medication time are selected,targeted,individualized treatment intervention,antihypertensive effect is remarkable,which has important clinical value.
