阻塞性睡眠呼吸暂停低通气综合征对老年高血压患者全天血压的影响

Obstructive sleep apnea hypopnea syndrome in elderly patients with hypertension effect of whole day blood pressure

目的分析阻塞性睡眠呼吸暂停低通气综合征对老年高血压患者全天血压的影响。方法选取2017年1月至2018年12月入住我院综合内科及护理中心的老年高血压患者194例,均行24小时动态血压及夜间多导睡眠图监测,测量身高体重及颈围,根据呼吸暂停低通气指数(AHI)分为四组:单纯高血压组50例,高血压合并轻度OSAHS组49例,高血压合并中度OSAHS组52例,高血压合并重度OSAHS组43例,对四组患者BMI、颈围及血压值进行比较。结果四组患者BMI、颈围、白天平均收缩压(dSBP)、白天平均舒张压(dDBP)、夜间平均收缩压(nSBP)、夜间平均舒张压(nDBP)、24 h平均收缩压(24hSBP)和24小时平均舒张压(24hDBP)相比,P<0.05,差异有统计学意义。结论随着OSAHS加重,老年高血压患者全天血压均明显升高,对于合并OSAHS的老年高血压患者,积极治疗OSAHS有助于血压更好达标。

Objective To analyze the effect of obstructive sleep apnea hypopnea syndrome on all-day blood pressure in elderly patients with hypertension.Methods USES January 2017 to December 2018.in our hospital comprehensive medical and nursing of elderly patients with high blood pressure in the center of the 194 cases,all line 24 hours ambulatory blood pressure monitoring and guide more sleep at night diagram,measurement of height and weight and neck circumference,according to apnea hypoventilation index(AHI)is divided into four groups:pure group of 50 cases of hypertension,hypertension with mild OSAHS group and 49 cases,52 cases of hypertension with moderate OSAHS group,hypertension with severe OSAHS group 43 cases,BMI,neck circumference and blood pressure in patients with four groups of values.Results BMI,neck circumference,mean daytime systolic blood pressure(dSBP),mean daytime diastolic blood pressure(dDBP),mean nocturnal systolic blood pressure(nSBP),mean nocturnal diastolic blood pressure(nDBP),mean 24-hour systolic blood pressure(24hSBP)and mean 24-hour diastolic blood pressure(24hDBP)of the four groups were compared,P<0.05,indicating statistically significant differences.Conclusion With the aggravation of OSAHS,the blood pressure of the elderly patients with hypertension is significantly increased throughout the day.
