Effect of early enteral nutrition combined with microecological preparation on gastrointestinal dysfunction in ICU patients with mechanical ventilation

摘要 目的探讨早期肠内营养结合微生态制剂对重症监护室(ICU)机械通气患者胃肠功能障碍的影响。方法选取2015年1月至2018年12月收治的入住ICU机械通气患者60例作为研究对象,采用数字表法随机均分为对照组30例和观察组30例。对照组采用早期肠内营养治疗,观察组采用早期肠内营养结合微生态制剂治疗,对两组患者实验室指标变化和胃肠功能障碍情况进行比较分析。结果治疗前两组白蛋白(Alb)、乳酸脱氢酶(LDH)、C反应蛋白(CRP)及白细胞计数(WBC)水平差异无统计学意义(P>0.05),治疗后观察组Alb水平比对照组明显更高,LDH、CRP及WBC比对照组明显更低(P<0.05)。治疗前两组胃肠功能评分比较无明显差异(P>0.05),治疗后观察组评分比对照组明显更低(P<0.05)。观察组胃肠道不良反应发生率为16.67%,比对照组(46.67%)显著更低(P<0.05)。结论对ICU机械通气患者采用早期肠内营养结合微生态制剂治疗能够有效维护机体内环境稳定,改善胃肠道功能状态,降低不良反应发生,值得推广。 Objective To investigate the effects of early enteral nutrition combined with microecological preparations on gastrointestinal dysfunction in patients with mechanical ventilation in intensive care unit(ICU).Methods 60 patients with ICU mechanical ventilation admitted from January 2015 to December 2018 were selected as subjects.They were randomly divided into control group(n=30)and observation group(n=30).The control group was treated with early enteral nutrition,and the observation group was treated with early enteral nutrition combined with microecological preparations.The changes of laboratory indexes and gastrointestinal dysfunction in the two groups were compared and analyzed.Results There were no significant differences in the levels of albumin(Alb),lactate dehydrogenase(LDH),C-reactive protein(CRP)and white blood cell count(WBC)between the two groups before treatment(P>0.05).The group was significantly higher,and LDH,CRP and WBC were significantly lower than the control group(P<0.05).There was no significant difference in gastrointestinal function score between the two groups before treatment(P>0.05).The score of the observation group was significantly lower than that of the control group after treatment(P<0.05).The incidence of gastrointestinal adverse reactions in the observation group was 16.67%,which was significantly lower than that of the control group 46.67%(P<0.05).Conclusion The use of early enteral nutrition combined with microecological preparations for ICU mechanical ventilation can effectively maintain the stability of the body environment,improve the functional status of the gastrointestinal tract,and reduce the occurrence of adverse reactions,which is worth promoting.
