芪参通冠汤辅助治疗冠心病稳定性心绞痛对患者血脂及血清tPAI-1、sICAM-1、MMP-9水平的影响

Effect of Qishen Tongguan Decoction Adjuvant Therapy on Levels of Blood Lipids and Serum tPAI-1,sICAM-1 and MMP-9 in Patients with Stable Angina Pectoris of Coronary Heart Disease

目的:观察芪参通冠汤辅助治疗冠心病稳定性心绞痛(SAP)对患者血脂及血清组织纤维蛋白溶解酶原激活物抑制剂-1(tPAI-1)、可溶性细胞间黏附分子-1(sICAM-1)和基质金属蛋白酶-9(MMP-9)水平的影响。方法:按随机数表法将94例冠心病SAP患者分为芪参通冠汤+阿托伐他汀钙组(研究组,n=47)和阿托伐他汀钙组(对照组,n=47)。于治疗前及治疗4周后检测2组患者血脂指标[总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)]、脂代谢指标[载脂蛋白A1(ApoA1)、载脂蛋白B(ApoB)]及血清学相关指标(tPAI-1、sICAM-1、MMP-9)水平,比较2组治疗前后中医症状积分差异。结果:治疗4周后,2组血清TC、TG、LDL-C、ApoB、tPAI-1、sICAM-1、MMP-9水平及中医症状积分均较治疗前降低,且研究组低于对照组(P<0.05);血清HDL-C、ApoA1水平均较治疗前升高,且研究组高于对照组(P<0.05)。结论:芪参通冠汤辅助治疗冠心病SAP有助于进一步改善患者血脂代谢和血液高凝状态,减轻血管内皮损伤,缓解疾病症状。

Objective: To observe the effect of Qishen Tongguan Decoction adjuvant therapy on levels of blood lipids and serum tissue plasminogen activator inhibitor-1(tPAI-1),soluble intercellular adhesion molecule-1(sICAM-1) and matrix metalloproteinase-9(MMP-9) in patients with stable angina pectoris(SAP) of coronary heart disease.Methods: 94 patients with coronary heart disease were divided into Qishen Tongguan Decoction + Atorvastatin Calcium group(study group,n=47) and Atorvastatin Calcium group(controlled group,n=47).The levels of blood lipids indexes [total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C)],lipid metabolism indicators [apolipoprotein A1(ApoA1),apolipoprotein B(ApoB)] and serology relevant indicators(tPAI-1,sICAM-1,MMP-9) were measured between the two groups before treatment and after 4 weeks of treatment.And the TCM symptoms scores were compared between the two groups before and after treatment.Results: After 4 weeks of treatment,the levels of serum TC,TG,LDL-C,ApoB,tPAI-1,sICAM-1 and MMP-9 and the TCM symptoms scores in the two groups were lower than those before treatment,and the levels and scores in the study group were lower than those in the controlled group(P < 0.05).The levels of serum HDL-C and ApoA1 were higher than those before treatment,and the levels in the study group were higher than those in the controlled group(P < 0.05).Conclusion: Qishen Tongguan Decoction for SAP of coronary heart disease can further improve the blood lipid metabolism and hypercoagulability,reduce vascular endothelial damage and alleviate the symptoms of disease.
