大黄清胰汤治疗重症急性胰腺炎临床观察

Clinical Observation of Using Dahuang Qingyi Decoction in the Treatment Severe Acute Pancreatitis

目的:观察中药大黄清胰汤治疗重症急性胰腺炎(SAP)的临床观察。方法:选取2015年2月~2018年2月我院收治的80例SAP患者为对象,采用随机数字法将其分为对照组和实验组各40例,对照组给予常规治疗,实验组在对照组的基础上给予中药大黄清胰汤治疗,比较两组治疗后的临床疗效、指标、淀粉酶、D-乳酸、二胺氧化酶(DAO)水平及血清炎性因子的变化。结果治疗前两组患者的淀粉酶、D-乳酸、DAO水平和血清TNF-α、CRP和IL-6水平和均无明显差异(P>0.05),治疗后实验组患者的淀粉酶、D-乳酸、DAO水平和血清TNF-α、CRP和IL-6水平均显著低于对照组,差异具有统计学意义(P<0.05);实验组的首次排便时间、腹痛、胀缓解时间、胃肠道功能恢复时间和住院时间均显著短于对照组,实验组总有效率为88.00%,明显高于对照组的82.22%,差异具有统计学意义(P<0.05)。结论:中药大黄清胰汤治疗SAP的效果显著,能够降低患者的淀粉酶、D-乳酸、DAO水平和血清TNF-α、CRP和IL-6水平,值得临床进一步推广。

Objective: To analyze the clinical effect of using Dahuang Qingyi Decoction in the treatment severe acute pancreatitis(SAP).Methods: 80 patients with SAP from February 2015 to February 2018 were divided into controlled group and experimental group by random number method.The controlled group was given routine treatment.The experimental group was given Dahuang Qingyi Decoction on the basis of the controlled group.The clinical efficacy,indexes and amylase of the two groups were compared,as well as D-lactate,two amine oxidase(DAO) level and serum inflammatory factors.Results: Before treatment,the levels of amylase,D-lactate,DAO and serum TNF-a,CRP and IL-6 were not significantly different between the two groups(P > 0.05).After treatment,the levels of amylase,D-lactate,DAO and serum TNF-a,CRP and IL-6 in the experimental group were significantly lower than those in the controlled group(P < 0.05).The first defecation time,abdominal pain,distension relief time,gastrointestinal function recovery time and hospitalization time of the experimental group were significantly shorter than those of the controlled group.The total effective rate of the experimental group was 88.00%,which was significantly higher than that of the controlled group(82.22%,P < 0.05).Conclusion: Using Dahuang Qingyi Decoction in treating SAP is effective.It can reduce the levels of amylase,D-lactic acid,DAO and serum TNF-a,CRP and IL-6.It is worthy of further clinical promotion.
