痔洗散熏蒸坐浴对混合痔术后水肿疼痛疗效观察

Effect of Hemorrhoids Fumigation Sitz Bath on Postoperative Edema and Pain for Patients with Mixed Hemorrhoid Surgery

目的:观察痔洗散熏蒸坐浴对混合痔手术患者术后水肿、疼痛的效果。方法:选取2015年1月~2017年12月我院肛肠外科收治并行手术治疗的混合痔患者84例,根据随机数字表分为观察组及对照组各42例,对照组给予3%高渗盐水熏蒸坐浴,观察组给予痔洗散熏蒸坐浴,治疗7天后,比较两组中医症候积分及疗效、不同时间点水肿及疼痛积分、症状(出血、水肿、疼痛)消失及创面愈合时间。结果:治疗7天后,两组中医症候积分均显著低于治疗前(P<0.05),且观察组降低幅度大于对照组(P<0.05);观察组症候疗效97.62%,显著高于对照组的80.95%(P<0.05);两组术后第4天水肿、疼痛积分均显著低于术后第1天(P<0.05),术后第8天水肿、疼痛积分均显著低于术后第1天及术后第4天(P<0.05),且观察组术后第4天,术后第8天水肿、疼痛积分均显著低于同时间点对照组(P<0.05);观察组出血、水肿、疼痛消失时间及创面愈合时间均显著低于对照组(P<0.05)。结论:痔洗散熏蒸坐浴用于混合痔术后患者中,可以改善患者中医症候,减轻水肿、疼痛程度,促进创面愈合。

Objective: To observe the effect of hemorrhoids fumigation sitz bath on postoperative edema and pain for patients with mixed hemorrhoid surgery.Methods: 84 cases of patients with mixed hemorrhoids who were treated with surgery in department of anus and intestine surgery from January 2015 to December 2017 were selected and divided into observation group and controlled group with 42 cases in each group according to the random number table method.The controlled group was treated with fumigation sitz bath of 3% hypeitonic saline,and the observation group was given hemorrhoids fumigation sitz bath.After 7 d of treatment,the TCM syndromes scores and efficacy,edema and pain points at different time points,symptoms disappearance(bleeding,edema,and pain) and wound healing time were compared between the two groups.Results: After 7 d of treatment,the TCM symptoms scores in the two groups were significantly lower than those before treatment(P < 0.05),and the reduction in the observation group was greater than that in the controlled group(P < 0.05).The symptomatic efficacy in the observation group was significantly higher than that in the controlled group(97.62% vs 80.95%)(P < 0.05).The edema and pain scores at 4 d after operation in the two groups were significantly lower than at 1 d after operation(P < 0.05).The edema and pain scores at 8 d after operation were significantly lower than those at 1 d and 4 d after operation(P < 0.05),and the edema and pain scores at 4 d and 8 d after operation in the observation group were significantly lower than in the controlled group at the same time point(P < 0.05).The disappearance times of bleeding,edema and pain and the wound healing time in the observation group were significantly lower than those in the controlled group(P < 0.05).Conclusion: Hemorrhoids fumigation sitz bath for patients with mixed hemorrhoids can improve TCM symptoms,reduce edema and pain,and promote wound healing.
