COPD患者肌肉抑制素、HCT、炎症细胞因子水平变化及其与肺功能的关系

The change of myostatin,HCT,inflammatory cytokine levels and its relationship with pulmonary function in the patients with COPD

目的探讨慢性阻塞性肺疾病(COPD)患者肌肉抑制素、红细胞压积(HCT)、炎症细胞因子水平变化及其与肺功能的关系。方法选取2017年10月一2018年3月汉中市西安交通大学附属汉中三二0一医院呼吸内科住院部收治的128例C0 PD患者作为病例组,其中COPD急性加重期患者作为AECOPD组(65例),COPD稳定期患者作为SCOPD组(63例),同期选取健康体检中心的60例健康体检者作为对照组。全部研究对象均检测肌肉抑制素、HCT、炎症细胞因子(TNF-α、PCT)与肺功能指标(FEV1%pred、FVC、PEF)。比较3组上述指标的差异,分析肌肉抑制素、HCT、TNF-α、PCT与FEVl%pred、FVC与PEF的关系。结果病例组患者肌肉抑制素、HCT、TNF-α与PCT水平明显高于对照组,FEV 1%pred、FVC与PEF水平明显低于对照组(P<0.05)。AEC0 PD组肌肉抑制素、HCT、TNF-α与PCT水平明显高于SC0 PD组,FEV l%pred、FVC与P E F水平明显低于SC0 PD组(P<0.05)。肌肉抑制素、HCT、TNF-α与肺功能指标呈显著负相关,PCT与FEV1%pred呈显著负相关(P<0.05)。结论C0PD患者肌肉抑制素、HCT、TNF-α、PCT水平明显升高,且上述指标与肺功能具有紧密的关系,其在指导临床治疗方案的制定中具有重要的意义。

Objective To study the The change of myostatin,hematokrit(HCT),inflammatory cytokine levels and its relationship with pulmonary function in the patients with chronic obstructive pulmonary diseases(COPD).Methods 128 patients COPD admitted in respiratory medicine inpatient department of our hospital during the period of October 2017 to March 2018 were selected as cases,acute exacerbations of COPD were as AECOPD group(65 cases),stable COPD were as SC0PD group(63 cases),at the same time,60 cases of healthy check-up from healthy check-up center were as control group.Levels of myostatin,HCT,inflammatory cytokine(TNF-α,PCT)and pulmonary function indexes(FEV1%pred,FVC,PEF)levels of all objects were detected.The difference of above indicators among groups were compared,the relationship between myostatin HCT,TNF-α,PCT and FEV1%pred,FVC,PEF were used Pearson correlation analysis.Results Levels of myostatin,HCT,TNF-α and PCT in the cases were significantly higher than there in the control group,levels of FEV1%pred,FVC,PEF were significantly lower than there in the control group,P<0.05.Levels of myostatin,HCT,TNF-α and PCT in the AECOPD group were obviously higher than there in the SCOPD group,levels of FEV1%pred,FVC,PEF were significantly lower than there in the SCOPD group,P<0.05.It had negative correlation between myostatin,HCT,TNF-α and pulmonary function indexes,it had negative correlation between PCT and FEV 1%pred,P<0.05.Conclusion Levels of myostatin,HCT,TNF-α and PCT increased significantly in the patients with COPD,and there is a close relationship between the above indexes and pulmonary function,which has vital significance in guiding clinical treatment plan formulation.
