N端B型钠尿肽前体和肌钙蛋白Ⅰ比值鉴别1型和2型心肌梗死的诊断价值研究

The value of using the N-terminal pro B-type natriuretic peptide and cardiac troponin I ratio in differentiating type 1 and 2 acute myocardial infarctions

目的研究使用N端B型钠尿肽前体和肌钙蛋白I比值鉴别1型和2型心肌梗死的诊断价值。方法回顾性分析2016年1月-2017年10月收入益阳市中心医院心血管内科的急性心肌梗死病例238人,根据冠脉造影结果和临床资料分为Ⅰ型心肌梗死和2型心肌梗死。收集并计算两组患者的N端B型钠尿肽前体和肌钙蛋白Ⅰ比值。分析该比值对鉴别I型和2型心肌梗死的诊断价值。结果N端B型钠尿肽前体和肌钙蛋白I比值鉴别2型心肌梗死的ROC曲线下面积为0.774,95%的可信区间为0.668-0.880。结论N端B型钠尿肽前体和肌钙蛋白I比值能较好的鉴别Ⅰ型和2型心肌梗死。

Objective To study the value of using the N-terminal pro B-type natriuretic peptide and cardiac troponin I ratio in differentiating typel and 2 acute myocardial infarctions.Methods The data of 238 patients with acute myocardial infarctions who were admilted to Yiyang Central Hospital from January 2016 to October 2017 were analyzed retrospectively.They were divided to type 1 and 2 acule myocardial infarctions groups by the coronary angiography and the clinic data.The results of iheir N-tnrniinal pro B-type natriuretic peptide and cardiac troponin I was collected.The ratio of them were calculated and the diagnostic value of the ratio was analyzed to differentiate type 1 and 2 acute myocardial infarctions.Results The area under llie curve(AUC)of receiver operating characteristic(ROC)curve for differentiating type 1 and 2 acute myocardial infarctions by the ratio of N-terminal pro B-type natriuretic peptide and cardiac troponin l was 0.774,the 95%confidence interval was 0.668 to 0.880.Conclusion The ratio of N-terminal pro B-type natriuretic peptide and cardiac troponin I can differentiate type land2acute myocardial infarctions properly.
