内镜黏膜下剥离术与腹腔镜手术治疗早期胃癌效果观察

Comparison of efficacy between endoscopic submucosal dissection and laparoscopic surgery for early gastric cancer

目的:对比内镜黏膜下剥离术与腹腔镜手术治疗早期胃癌的效果。方法:2013年3月—2016年10月早期胃癌患者88例,随机分成研究组与对照组各44例。对照组采用腹腔镜手术治疗,研究组采用内镜黏膜下剥离术治疗。比较两组临床疗效,各项手术指标,手术前后血清TNF-α、IL-6、CRP水平,术后并发症。结果:研究组总有效率97.73%(43/44),明显高于对照组的84.09%(37/44);研究组各项手术指标水平均明显低于对照组;两组手术后血清TNF-α、IL-6、CRP水平均明显高于手术前;研究组术后并发症发生率为2.27%(1/44),明显低于对照组的18.18%(8/44)。结论:内镜黏膜下剥离术对早期胃癌患者疗效更理想,减少术后并发症。

Objective:To compare and analyze the efficacy of endoscopic submucosal dissection and laparoscopic surgery in the treatment of early gastric cancer.Methods:88 patients with early gastric cancer who were treated in our hospital from March 2013 to October 2016 were selected as observation objects.They were divided into study group and control group by random drawing.The control group was treated with laparoscopic surgery,and the study group was treated with endoscopic submucosal dissection.The clinical efficacy,operative index level,serum TNF-α,IL-6,CRP levels and postoperative complications were compared between the two groups.Results:The total effective rate of treatment in the study group was 97.73%(43/44),which was significantly higher than the control group of 84.09%(37/44);The time of operation,the amount of bleeding,the time of hospitalization and the cost of medical treatment in the study group were significantly lower than the control group;The serum levels of TNF-α,IL-6 and CRP in the two groups were significantly higher than before the operation;The complication rate of study group was 2.27%(1/44),which was significantly lower than the control group of 18.18%(8/44).The difference was statistically significant(P<0.05).Conclusion:Endoscopic submucosal dissection in the treatment of patients with early gastric cancer has more effective and can effectively promote the recovery of patients and reduce postoperative complications,which is worthy of clinical application.
