个体化肠内外营养对颈髓损伤伴低钠血症患者的干预效果分析

Effect of individualized enteral-parenteral nutrition on cervical spinal cord injury with hyponatremia

目的:研究个体化肠内肠外营养对颈髓损伤伴低钠血症患者营养状态及生活质量的影响。方法:选取2018年1至2019年3月河北医科大学第三医院收治的颈髓损伤并发低钠血症患者86例为研究对象,按随机数字表法分为观察组和对照组,每组43例。对照组行常规饮食干预,观察组在对照组的基础上实施个体化肠内肠外营养干预。比较两组干预前、后心率、尿钠及血钠水平、营养指标[血清总蛋白(TP)、白蛋白(Alb)、前白蛋白(PA)、转铁蛋白(TRF)、血红蛋白(Hb)]、颈椎功能评价量表(JOA)和生活质量量表(SF-36)评分。结果:观察组干预后JOA评分上肢运动功能、下肢运动功能、感觉和膀胱功能评分、心率、血钠及SF-36评分均显著高于干预前及对照组,尿钠、TP、Hb、PA、Alb、TRF均显著低于干预前及对照组(P<0.05)。结论:实施个体化肠内肠外营养方案对颈髓损伤并发低钠血症患者进行干预疗效显著,有助于患者营养状态的改善,可有效提高患者生活质量。

Objective:To study the effect of individualized enteral-parenteral nutrition on nutritional status and quality of life of patients with cervical spinal cord injury and hyponatremia.Methods:86 patients with cervical spinal cord injury complicated with hyponatremia admitted to our hospital from January 2018 to March 2019 were selected,and randomly divided into an observation group and a control group,with 43 cases in each group.The patients in the control group received conventional dietary intervention,and those in the observation group were treated as the control group with addition of individualized enteral-parenteral nutrition intervention.The Japanese Onhopedic Association(JOA)score,short form 36 questionnaire(SF-36),heart rate,urinary sodium,blood sodium,serum total protein(TP),albumin(Alb),prealbumin(PA),transferrin(TRF)and hemoglobin(Hb)levels were compared.Results:The JOA scores of upper limb motor function,lower limb motor function,sensation and bladder function score,heart rate,serum sodium and SF-36 score in the observation group were higher than those before intervention and control group,while the urine sodium,TP,Hb,PA,Alb and TRF levels were significantly lower(P<0.05).Conclusion:Individualized enteral-parenteral nutrition therapy application in patients with cervical spinal cord injury and hyponatremia was significantly effective,and it was helpful to the improvement of nutrition state and life quality of patients.
