仰卧间歇角度牵引治疗神经根型颈椎病的临床疗效分析

Clinical efficacy of supine intermittent angle traction in the treatment of cervical spondylotic radiculopathy

目的:探讨仰卧间歇角度牵引治疗神经根型颈椎病(CSR)患者的临床疗效及对椎间孔容积的影响。方法:选取2017年4月至2018年7月黑龙江中医药大学附属第二医院哈南分院骨科收治的148例CSR患者为研究对象,按随机数字表法分为观察组和对照组,每组74例。对照组行常规治疗,观察组在对照组的基础上加用仰卧间歇角度牵引。比较两组临床疗效、颈椎JOA量表日本骨科协会评分、椎间孔容积及治疗期间并发症发生情况。结果:观察组临床疗效总有效率(91.89%)显著高于对照组(78.38%)(P<0.05);观察组治疗后JOA评分显著高于治疗前及对照组治疗后(P<0.05);观察组在治疗第3天、第7天、第14天后,椎间孔容积显著大于治疗前及对照组治疗后(均P<0.05);观察组并发症总发生率(10.81%)与对照组(13.51%)比较,差异无统计学意义(P>0.05)。结论:仰卧间歇角度牵引治疗CSR患者具有较好的临床疗效,可有效改善患者病情,且安全性较高,值得临床推广。

Objective:To investigate the clinical efficacy of supine intermittent angle traction in the treatment of cervical spondylotic radiculopathy(CSR)and its effect on the volume of intervertebral foramen.Methods:148 patients with CSR admitted to our hospital from April 2017 to July 2018 were selected,and randomly divided into an observation group and a control group,with 74 cases in each group.The patients in the control group received routine treatment,while those in the observation group were treated as the control group with addition of supine intermittent angle traction.The clinical efficacy,cervical Japanese Orthopaedic Association scale(JOA)score,intervertebral foramen volume and the incidence of complications were compared.Results:The total effective rate of the observation group was 91.89%,which was significantly higher than that of the control group(78.38%)(P<0.05).After treatment,the JOA score of the observation group was significantly higher than that of the control group,and the volume of intervertebral foramen in the observation group was significantly higher after 3,7 and 14 days of treatment(P<0.05).The total incidence of complications was similar between the two groups(P>0.05).Conclusion:Supine intermittent angle traction was effective in treating CSR,and it could effectively improve the patient's condition with a high safety.
