血清神经元特异性烯醇化酶在表现为精神障碍的尿毒症脑病诊断中的意义研究 被引量:1

摘要 目的:研究血清神经元特异性烯醇化酶(neuron-specific enolase,NSE)在表现为精神障碍的尿毒症脑病诊断中的意义。方法:选取2016年01月~2019年01月来我院住院的患者,根据入组标准分为尿毒症脑病组、尿毒症组、功能性精神病组,每组各40例,均抽取外周静脉血送我院生化室行血清NSE检测,并用阴性和阳性症状量表(PANSS)对尿毒症脑病组患者的精神症状进行评分。结果:尿毒症脑病患者NSE明显升高,而尿毒症患者、功能性精神病组患者血清NSE均在正常范围,尿毒症脑病组血清NSE与尿毒症组及功能性精神病组相比差异有统计学意义,尿毒症组与功能性精神病组相比NSE差异无统计学意义,pearson相关系数分析提示尿毒症脑病组NSE升高的水平与患者精神异常的严重程度评分PANSS评分呈显著正相关。结论:尿毒症脑病患者神经元特异烯醇化酶NSE明显升高,其升高的水平与患者精神异常的严重程度评分PANSS评分呈显著正相关,或可作为尿毒症脑病早期诊断、鉴别诊断及判断脑病的严重程度的特异性因子。 Objective:To study the significance of serum neuron-specific enolase(NSE)in the diagnosis of uremic encephalopathy with mental disorders.Methods:Patients admitted to our hospital from January 2016 to January 2019 were enrolled according to the inclusion criteria.They were divided into uremic encephalopathy group,uremia group and functional psychiatric group.Each group had 40 cases,and peripheral venous blood was taken.Serum NSE was performed in the biochemical room of our hospital,and the psychiatric symptoms of the patients with uremia encephalopathy were scored using the negative and positive symptom scale(PANSS).Results:Serum NSE was significantly elevated in patients with uremia encephalopathy,and serum NSE in uremic patients and functional psychiatric patients was in the normal range.Serum NSE in uremic encephalopathy group was significantly different from uremic group and functional psychiatric group.There was no significant difference in NSE between the uremic group and the functional psychiatric group.The pearson correlation coefficient analysis showed that the level of NSE in the uremic encephalopathy group was significantly positively correlated with the PANSS score of the patient's mental abnormality score.Conclusion:Serum NSE in patients with uremia encephalopathy is significantly increased,and the elevated level is significantly positively correlated with the PANSS score of the patient's mental abnormality score,may be used as a specific factor for early diagnosis,differential diagnosis and determination of the severity of encephalopathy in uremic encephalopathy.
