摘要
目的:探讨对冠心病心肌缺血实施MRI心肌灌注成像的诊断价值及其对患者睡眠的影响。方法:选取2018年1月至2019年5月厦门大学附属第一医院收治的冠心病患者40例作为观察组,另选择同期本院门诊的40例健康体检人员为对照组。2组患者均进行MRI心肌灌注成像及冠状造影检查,分析诊断效果。结果:观察组的首过灌注时间、延迟强化信号强度高于对照组,最大上升斜率低于对照组(P<0.05)。实施MRI心肌灌注成像对左前降支病变诊断敏感性为96.0%,特异性为93.3%,准确性为95.0%;对右冠状动脉病变的诊断敏感性为88.9%,特异性为91.8%,准确性为85.0%;对左旋支病变的诊断敏感性为93.3%,特异性为88.0%,准确性为90.0%。冠心病组患者检查前与检查后的睡眠质量评分差异无统计学意义(P>0.05)。结论:对冠心病心肌缺血实施MRI心肌灌注成像诊断效果良好,对患者睡眠质量无影响。
Objective:To evaluate the diagnostic value of MRI myocardial perfusion imaging in myocardial ischemia of coronary heart disease(CAD)and its effect on sleep quality.Methods:From January 2018 to May 2019,40 patients with coronary heart disease treated in the first affiliated Hospital of Xiamen University were selected as observation group,and 40 healthy persons in outpatient department of our hospital were selected as control group.MRI myocardial perfusion imaging and coronary angiography were performed in both groups,and the diagnostic effect was analyzed.Results:The first over-perfusion time and delay-reinforced signal strength of the observation group were higher than those of the control group,and the maximum upward slope was lower than that of the control group(P<0.05).The diagnostic sensitivity of MRI cardiomyopathy imaging was 96.0%,the specificity was 93.3%,and the accuracy was 95.0%.The diagnostic sensitivity of right coronary artery lesions was 88.9%,the specificity was 91.8%,and the accuracy was 85.0%.The diagnostic sensitivity of l-spin lesions was 93.3%,the specificity was 88.0%and the accuracy was 90.0%.There was no significant difference in sleep quality score between patients with coronary heart disease before and after examination(P>0.05).Conclusion:And the imaging diagnosis effect on myocardial ischemia of the coronary heart disease is good,and it has no effect on patient sleep quality.
