机械通气对重症监护患者睡眠影响的研究

Effects of Mechanical Ventilation on Sleep in Intensive Care Patients

目的:观察机械通气对重症监护患者睡眠质量的影响。方法:选取2017年10月至2018年10月三明市第二医院收治的重症监护患者60例作为研究对象,根据患者通气方法的不同分为对照组、观察组1、观察组2,每组20例。对照组给予自主呼吸,面罩吸氧,未给予镇静。观察组1给予气管插管下机械通气,未给予镇静。观察组2给予气管插管下机械通气,同时予以镇静。结果:观察组2通气10 min后HR(86.1±1.2)次/min、RR(18.5±1.6)次/min、Ramsay(3.5±0.1)分、PSQI(5.1±0.3)分、1期睡眠占0、2期睡眠占15%、睡眠障碍发生率15%、VAS(2.0±0.2)分、撤机时间(3.1±0.3)d、住ICU(4.2±0.3)d、住院(6.0±0.3)d,与观察组1比较,差异有统计学意义(P<0.05)。结论:机械通气可导致重症监护患者发生睡眠障碍,机械通气的同时给予镇静,可改善睡眠质量。

Objective:To observe the effect of mechanical ventilation on sleep quality in intensive care patients.Methods:A total of 60 cases of intensive care patients admitted to the second sanming hospital from October 2017 to October 2018 were selected and divided into control group,observation group 1 and observation group 2 according to the different ventilation methods of the patients,20 cases in each group.The control group was given spontaneous breathing,mask oxygen,no sedation.Observation group 1 was given mechanical ventilation under endotracheal intubation without sedation.Observation group 2 was given mechanical ventilation and sedation under endotracheal intubation.Results:The observation group 2 ventilation after 10 min HR(86.1±1.2)times/min,RR(18.5±1.6)times/min,Ramsay(3.5±0.1)points,PSQI(5.1±0.3)points,stage 1 sleep accounted for 0,2 sleep accounted for 15%15%,the incidence of sleep disorders,VAS(2.0-0.2),the withdraw machine time(3.1±0.3)d,live in ICU(4.2±0.3)(6.0±0.3)d d and hospitalization,and compared with the observation group 1,significant difference(P<0.05).Conclusion:Mechanical ventilation can lead to sleep disturbance in intensive care patients.
