呼吸机辅助呼吸终期阶段的医学伦理决策问题辨析

Discrimination and Analysis of Medical Ethical Decision-making in the End-Stage of Respiration Assisted by Ventilator

呼吸机辅助呼吸终期阶段的治疗在临床上常用于呼吸系统处于终末状态的患者,处于此治疗阶段的患者,通常存在明显的不良心理状态,同时承受着巨大的身体痛苦,生存质量较低,而患者家庭也承受着巨大的精神和经济压力。如何更好地维护患者接受治疗的权利,如何处理患者治疗与家庭经济压力的关系,如何缓解患者治疗期间的心理问题,这些都是呼吸机辅助呼吸终期阶段医学伦理学方面要解决的问题。在对上述问题进行分析的基础上,提出了如下思考:重视患者心理动态、增加医患沟通,重视人文关怀在呼吸机辅助通气患者中的应用,兼顾患者利益、他人利益和社会公益。

The treatment of ventilator assisted the end stage of respiration is often used in the clinical practice for patients whose respiratory system is in the end state.In this stage of treatment,patients usually have the obvious terrible psychological state,suffer the huge physical pain and have the poor quality of life.And the families of patients are also under the great mental and economic pressure.These are some problems to be solved in the medical ethics of ventilator assisted he end stage of respiration,such as how to better protect the right of patients to receive treatment,how to deal with the relationship between patients’treatment and families’economic pressure,and how to alleviate the psychological problems of patients during treatment.Based on the analysis of this problems,this study put forward the following thoughts,including:attaching importance to patients’psychological dynamics,increasing the doctor-patient communication,attaching importance to the application of humanistic care in patients with ventilator assisted respiration,taking into account the interests of patients,others and social public welfare.
