Source and Course of Gastroesophageal Reflux Disease and Its Treatment

胃食管反流病是临床常见病,以白金甘胃汤为基础方,设立3个证型:胆胃郁热或胆胃不和证、脾胃虚寒证、寒热错杂证。热以肝胆热扰胃府为主,寒则脾胃虚寒水谷运化无权,寒热错杂则在于肝火脾虚共存相互戕害胃府为主。胆胃郁热证以白金甘胃汤加小柴胡汤治疗;脾胃虚寒证则以白金甘胃汤加香砂六君子汤治疗;寒热错杂证则用白金甘胃汤加乌梅丸治疗。临证可配合腹部"三六按摩"法、"胃肠乐"肚脐贴等中医外治法。

Gastroesophageal reflux disease is a common clinical disease.Based on Baijin Ganwei Decoction,three syndrome types are established as follows:Gallbladder and Stomach Heat stagnation Syndrome or disharmony between Gallbladder and Stomach Syndrome,Spleen and Stomach deficiency and Coldness Syndrome as well as Coldness and Hotness Mixture Syndrome.The Heat is mainly caused by the Liver and Gallbladder heat,which troubles the Stomach;the Cold means the Spleen and Stomach deficiency and Coldness,which cannot transform and digest the foods;the Coldness and Hotness Mixture means coexisting of the Liver Fire and the Spleen deficiency,which injures the Stomach.In treatment of Gallbladder and Stomach Heat stagnation Syndrome,Baijin Ganwei Decoction plus Xiao Chaihu Decoction can be used;in treatment of Spleen and Stomach deficiency and Coldness Syndrome,Baijin Ganwei Decoction plus Xiangsha Yangwei Decoction can be used;in treatment of Coldness and Hotness Mixture Syndrome,Baijin Ganwei Decoction plus Wumei Pill can be used.In clinical practices,the prescription can be used with the"threesix massage"method of the abdomen or Wei Chang Le Duqi Paste and other traditional Chinese medicine external therapy.
