摘要
目的探讨超声引导下肩峰下滑囊和结节间沟注射联合功能锻炼治疗肩周炎的临床效果。方法纳入2018年3月至2019年3月在我院治疗的112例肩周炎患者,进行超声引导下肩峰下滑囊联合结节间沟注射消炎镇痛液同时结合功能锻炼,比较治疗前与治疗后2周、4周的VAS评分、肩关节主动前屈(AFROM)和外展活动度(AAROM)。结果超声引导下肩峰下滑囊联合结节间沟注射消炎镇痛药物治疗前VAS评分为(8.20±0.32)分,治疗后2、4周评分分别为(1.86±0.12)分、(0.78±0.13)分,分别与治疗前相比,差异均有统计学意义(P<0.01);肩关节前屈活动度治疗前为(69.80±3.52)°,治疗后2、4周分别为(1.86±0.12)°和(0.78±0.13)°;肩关节外展活动度治疗前为(72.07±1.56)°,治疗后2、4周分别为(161.33±2.24)°、(175.35±2.18)°,治疗后肩关节前屈及外展活动度增加,差异均有统计学意义(P<0.01)。结论超声引导下肩峰下滑囊联合结节间沟注射消炎镇痛液治疗联合肩关节功能锻炼可有效改善肩周炎患者的疼痛程度和肩关节活动范围。
Objective To investigate the clinical effect of ultrasound-guided subacromial bursa and intertubercularsulcus injection combined with functional exercise in the treatment of scapulohumeral periarthritis.Methods112 patientswith scapulohumeral periarthritis during March in 2018 to March in 2019 were received the treatment of drug injection intosubacromial bursa and intertubercular sulcus injection under ultrasound guidance and functional exercise. Visual analoguescale(VAS),active flexion range of motion(AFROM)and active abduction range of motion(AAROM)were used to evaluatethe status of scapulohumeral periarthritis before treatment,2 weeks and 4 weeks after treatment.ResultsAfter treatment,the VAS scores were significantly decreased at the end of the second week(1.86±0.12)and the fourth week(0.78±0.13)than those before treatment(8.20±0.32),and the differences were significant(P<0.01). Compared with the AFROM andAAROM of shoulder joint before treatment(69.80±3.52°),the AFROM and AAROM were significantly increased at theend of the second week(1.86±0.12°)and the fourth week(0.78±0.13°),and the differences were significant(P<0.01).ConclusionUltrasound-guided injection of anti-inflamatory and analgesic liqid in subacromial bursa and intertubercularsulcus combined with functional exercise can effectively improve the degree of pain and the range of motion of shoulder jointin scapulohumeral periarthritis patients.
