多层螺旋CT在胃肠道间质瘤术前诊断中的临床意义

Clinical significance of multi-slice spiral CT in preoperative diagnosis of gastrointestinal stromal tumor

目的探讨多层螺旋CT运用于胃肠道间质瘤患者术前诊断中的临床价值。方法选择本院2016年4月至2018年12月收治的疑似胃肠道间质瘤患者30例作为研究对象,对患者均实行超声检查、多层螺旋CT检查,分析检查结果。结果 30例患者经病理检查,胃肠道间质瘤占56.67%,非胃肠道间质瘤占43.33%;胃肠道间质瘤患者低回声例数、伴存后面声影例数、内部血流较为丰富例数、界限不清楚例数明显多于非胃肠道间质瘤患者(P<0.05);胃肠道间质瘤患者增强起始时间、到达峰值时间显著高于非胃肠道间质瘤患者(P<0.01),胃肠道间质瘤患者强度半降时间、曲线下面积、增强强度值显著低于非胃肠道间质瘤患者(P<0.01);多层螺旋CT诊断胃肠道间质瘤的特异度、敏感度显著高于超声检查(P<0.01)。结论在胃肠道间质瘤患者术前诊断中采用多层螺旋CT检查的临床效果较好,诊断特异度、敏感度均比较高。

Objective To investigate the clinical value of multi-slice spiral CT in the preoperative diagnosis of patients with gastrointestinal stromal tumors. Methods 30 patients with suspected gastrointestinal stromal tumors who were treated in our hospital from April 2016 to December2018 were included in the project data. Ultrasound examination and multi-slice spiral CT examination were performed on the patients. Results 30 patients with pathological examination, gastrointestinal stromal tumors accounted for 56.67%, non-gastrointestinal stromal tumors accounted for43.33%;gastrointestinal stromal tumor patients with low echo cases, accompanied by the number of subsequent sound shadow cases, the number of internal blood flow was more abundant, and the number of unclear cases was significantly higher than that of non-gastrointestinal stromal tumors(P<0.05). The increase of initial time and peak time of patients with gastrointestinal stromal tumors was significantly higher than that of non-stomach in patients with intestinal stromal tumors(P<0.01), the half-down time, area under the curve and the intensity of enhancement of gastrointestinal stromal tumor patients were significantly lower than those of non-gastrointestinal stromal tumors(P<0.01). The specificity and sensitivity of spiral CT in the diagnosis of gastrointestinal stromal tumors were significantly higher than those in ultrasound examination(P<0.01). Conclusion The multislice spiral CT examination in the preoperative diagnosis of gastrointestinal stromal tumor patients has a good clinical effect, and the diagnostic specificity and sensitivity are relatively high.
