不同麻醉方法用于阴式全子宫切除麻醉的效果

Different Anesthesia Methods for the Effect of Vaginal Hysterectomy

目的研究分析在阴式全子宫切除患者中运用不同麻醉方式的麻醉效果差异。方法将2018年4月—2019年1月在本院行阴式全子宫切除术患者118例设为研究对象,经电脑随机法分组为观察组(59例)以及对照组(59例)。对照组患者接受硬膜外麻醉,观察组患者接受腰硬联合麻醉。分析两组患者麻醉前后的心率、血压变化以及麻醉效果。结果两组患者的在麻醉后的心率、舒张压以及收缩压水平均显著较麻醉前降低(P<0.05),而麻醉后观察组的心率、舒张压与对照组差异均不存在统计学意义(P>0.05),而收缩压显著高于对照组(P<0.05)。观察组中患者的麻醉起效时间、阻滞完善时间以及术中出血量均显著低于对照组(P<0.05),手术时间显著长于对照组(P<0.05)。结论临床上开展阴式全子宫切除期间采用腰硬联合麻醉对患者的血压影响较小,麻醉效果更好,起效快,手术出血量较低,具有较高的临床价值。

Objective To study and analyze the anesthetic effects of different anesthesia methods in patients undergoing vaginal hysterectomy.Methods 118 patients who underwent vaginal hysterectomy in our hospital from April 2018 to January 2019 were randomly divided into observation group(59 cases)and control group(59 cases).Patients in the control group received epidural anesthesia,while patients in the observation group received combined spinal-epidural anesthesia.The changes of heart rate,blood pressure and anesthetic effect before and after anesthesia were analyzed.Results The heart rate,diastolic blood pressure and systolic blood pressure of the two groups after anesthesia were significantly lower than those before anesthesia(P<0.05),while the heart rate and diastolic blood pressure of the observation group after anesthesia were not significantly different from those of the control group(P>0.05),but the systolic blood pressure was significantly higher than that of the control group(P<0.05).The onset time of anesthesia,the perfect time of block and the amount of bleeding during operation in the observation group were significantly lower than those in the control group(P<0.05),and the operation time was significantly longer than that in the control group(P<0.05).Conclusion Combined spinal-epidural anesthesia during vaginal hysterectomy has less effect on blood pressure,better anesthesia effect,faster onset,lower operative bleeding,and higher clinical value.
