协同护理模式对高龄产妇分娩结局的影响

Effect of Collaboration Care Model on Delivery Outcomes in Elderly Pregnant Women

目的:探讨协同护理模式对高龄产妇分娩结局的影响。方法:选择2014年1月-2017年12月就诊于南京市扬子医院的高龄待产妇146例作为研究对象,将其随机分为对照组(n=73)和干预组(n=73)。对照组给予产科常规护理,干预组在产科常规护理基础上给予协同护理模式。比较两组产程、剖宫产率、产后2 h出血量、新生儿Apgar评分及心理状态。结果:干预组剖宫产率为28.8%,低于对照组的45.2%,差异有统计学意义(P<0.05)。干预组新生儿Apgar评分为(8.7±0.7)分,显著高于对照组的(7.5±0.6)分,差异有统计学意义(P<0.05)。干预组第二产程时间(0.8±0.2)h,第三产程时间(5.8±1.3)min,产后24 h SAS评分(43.5±4.9)分,产后24 h SDS评分(44.4±5.3)分,均低于对照组的(1.1±0.4)h、(7.1±1.5)min、(55.9±6.2)分,(60.2±8.7)分,差异均有统计学意义(P<0.05)。两组产后2 h出血量及第一产程时间比较差异无统计学意义(P>0.05)。结论:协同护理模式可有效改善高龄产妇分娩结局。

Objective:To study the effect of collaboration care model on delivery outcomes in elderly pregnant women.Method:From January 2014 to December 2017,146 elderly pregnant women admitted in Nanjing Yangzi Hospital were selected as study subjects.They were randomly divided into the control group(n=73)and the intervention group(n=73).The control group was given routine obstetric care,and the intervention group was given collaboration care model on the basis of routine obstetric care.The labor process,cesarean section rate,amount of bleeding after 2 h of labor,neonatal Apgar score and mental status were compared between two groups.Result:The cesarean section rate in the intervention group was 28.8%,which was lower than 45.2%in the control group,and the difference was statistically significant(P<0.05).Apgar score in the intervention group was(8.7±0.7)points,which was significantly higher than(7.5±0.6)points in the control group,the difference was statistically significant(P<0.05).The time of second labor process(0.8±0.2)h,the time of third labor process(5.8±1.3)min,the SAS score(43.5±4.9)points at 24 h postpartum,the SDS score(44.4±5.3)points at 24 h postpartum in the intervention group were all lower than(1.1±0.4)h,(7.1±1.5)min,(55.9±6.2)points,and(60.2±8.7)points in the control group,the differences were statistically significant(P<0.05).There were no significant differences in the bleeding after 2 h of labor and the time of the first labor process between the two groups(P>0.05).Conclusion:The collaboration care model can improve the delivery outcomes in elderly pregnant women.
