超微经皮肾镜和输尿管软镜在治疗肾结石中的感染控制及效果对比

Comparison of Infection Control and Effect of Ultramicroscopic Percutaneous Nephrolithotomy and Ureteroscopy Lithotripsy in the Treatment of Kidney Stones

目的:研究肾结石患者接受超微经皮肾镜碎石术(UPCNL)及输尿管软镜碎石术(URSL)治疗对感染控制及效果的影响。方法:以2017年5月-2019年5月在笔者所在医院实施手术治疗的100例肾结石患者作为观察对象,将其按照随机数字表法分为两组,各50例。A组予以URSL治疗,B组予以UPCNL治疗。比较两组手术疗效、手术时间、住院时间及感染控制情况。结果:B组手术总有效率显著高于A组,差异有统计学意义(P<0.05)。B组手术时间、住院时间、术中出血量均显著优于对照组,差异有统计学意义(P<0.05)。两组血WBC>10×10^9/L、体温>38.5℃、SIRS率比较,差异无统计学意义(P>0.05)。结论:UPCNL与URSL对肾结石患者术后感染的控制效果均较好,但UPCNL整体手术处理效果更理想,值得推荐。

Objective:To investigate the effect of ultrastructural percutaneous nephrolithotomy(UPCNL)and ureteroscopy(URSL)on infection control and efficacy in patients with kidney stones.Method:A total of 100 patients with kidney stones who underwent surgery in our hospital from May 2017 to May 2019 were selected as the objects of observation.According to the random number table method,the patients were divided into two groups,with 50 cases in each group.Group A was treated with URSL.Group B was treated with UPCNL.The surgical efficacy,operation time,length of hospital stay,and infection control related indicators were compared between the two groups.Result:The total effective rate in group B was significantly higher than that in group A,the difference was statistically significant(P<0.05).The operation time,length of hospital stay and intraoperative blood loss in group B were significantly better than those in the control group,the differences were statistically significant(P<0.05).There were no significant differences in blood WBC>10×10^9/L,body temperature>38.5℃and SIRS rate between the two groups(P>0.05).Conclusion:Both UPCNL and URSL have better control effects on postoperative infection in patients with kidney stones,but the overall surgical treatment effect of UPCNL is more ideal and worthy of recommendation.
