旁入穿刺法细针腰麻用于剖宫产手术的临床效果分析

Clinical Effect Analysis of Fine Needle Lumbar Anesthesia for Cesarean Section by Bypass Puncture

目的研究旁入穿刺法细针腰麻对剖宫产手术的临床效果。方法选取2016年3月—2019年3月行剖宫产手术的60例患者为研究对象,将其随机均分为研究组与对照组,研究组采用25 G笔尖式细针腰麻,并结合重比重局部麻醉的手术方式,对照组采用传统斜面针腰麻。对比两组患者基础资料、腰麻成功率、腰麻效果发生时间与Bromage评分阻滞平面情况、术后并发症发生率。结果两组患者手术有关基础资料对比,差异无统计学意义(P>0.05),研究组首次及整体成功率高于对照组(P<0.05),两组患者Bromage评估分值为1、2分的起效时间与腰麻平均数未存在明显差异(P>0.05),研究组最高阻抑平面明显优于对照组(P<0.05),对照组术后头痛、腰痛发生率明显高于研究组(P<0.05)。结论剖宫产手术选择旁入穿刺法细针腰麻,具有术后并发症发生率较低,穿刺成功率较高的优点,建议在临床推广使用。

Objective To study the clinical effect of fine needle lumbar anesthesia in cesarean section by bypass puncture.Methods Sixty patients who underwent cesarean section from March 2016 to March 2019 were enrolled in the study.They were randomly divided into study group and control group.The study group used 25 G pen tip fine needle spinal anesthesia,combined with heavy weight.For local anesthesia,the control group used traditional beveled spinal anesthesia.The baseline data,spinal anesthesia success rate,spinal anesthesia effect time,Bromage score block,and postoperative complication rate were compared between the two groups.Results There was no significant difference in the basic data of surgery between the two groups(P>0.05).The first and overall success rate of the study group was higher than that of the control group(P<0.05).The Bromage evaluation scores of the two groups were 1,2 points.There was no significant difference between the onset time and the average number of spinal anesthesia(P>0.05).The highest inhibition plane in the study group was significantly better than the control group(P<0.05).The incidence of headache and low back pain in the control group was significantly higher than that in the study group(P<0.05).Conclusion The cesarean section chooses the fine needle lumbar anesthesia by the puncture method.It has the advantages of lower postoperative complications and higher puncture success rate.It is recommended to be used in clinical practice.
