自制乳胶水袋结合针刺疗法对早产儿50例扁头综合征发生率及其预后的影响

Effect of self-made latex water bag combined with acupuncture on the incidence and prognosis of DPB in premature infants

目的观察自制乳胶水袋结合针刺疗法对早产儿扁头综合征发生率及其预后改善的影响,并探讨其临床应用价值。方法选取2016年8月1日至2017年7月31日于丽水市人民医院新生儿重症监护病房(NICU)住院的早产儿100例,按随机数字表法分为对照组和治疗组,各50例。对照组采用传统软枕干预方案,治疗组采用自制乳胶水袋干预睡姿方案及针刺治疗。根据扁头综合征分级评估依据,对早产儿扁头综合征的发生率进行评估及组间死亡率进行比较。随访出生后1年新生儿的生存状况和死亡率。结果治疗组扁头综合征发生率为6.0%,显著低于对照组的30.0%,2组比较,差异有统计学意义(P<0.05)。治疗组斜头畸形发生率和平头畸形发生率分别为4.0%和2.0%,均显著低于对照组的16.0%和14.0%,差异有统计学意义(P<0.05)。出生后1年,治疗组新生儿无死亡,对照组死亡5例,2组比较,差异有统计学意义(P<0.05)。结论自制乳胶水袋结合针刺干预在早产儿头颅生长中临床意义显著,对降低新生儿出生后扁头综合征发生率及改善生存率具有重要意义。

Objective To observe the effect of self-made latex water bag combined with acupuncture on the incidence of deformational plagiocephaly and/or brachycephaly(DPB)syndrome and its prognosis in premature infants and to explore its clinical application value.Methods A total of 100 premature infants admitted to the Neonatal Intensive Care Unit(NICU)of Lishui the People’s Hospital from August 1,2016 to July 31,2017 were divided into control group and treatment group according to random number table method,with 50 cases in each.The control group used the traditional soft pillow intervention program,and the treatment group used the self-made latex water bag to intervene in the sleeping position program and acupuncture intervention.According to the grading assessment of DPB syndrome,the incidence of DPB syndrome in premature infants was evaluated and the intergroup mortality was compared.The survival status and mortality of newborns were recorded in the one year of follow-up.Results The incidence of DPB syndrome in the treatment group was 6.0%,which was significantly lower than 30.0%in the control group,and the difference between the two groups was statistically significant(P<0.05).The incidence of plagiocephaly and brachycephaly in the treatment group were 4.0% and 2.0%,respectively,which were significantly lower than 16.0%and 14.0% in the control group(P<0.05).After one year follow-up,there was no death in the treatment group and 5 deaths in the control group,and the difference was statistically significant between the two groups(P<0.05).Conclusion The self-made latex water bag combined with acupuncture intervention has marked clinical significance in the skull growth of premature infants,which is of great significance in reducing the incidence of DPB syndrome and improving survival rate in neonates.
