摘要
目的采用受试者工作特征曲线(ROC)探讨血清8-羟基脱氧鸟苷酸(8-OHDG)和总超氧化物歧化酶(T-SOD)检测在脑卒中及脑出血早期诊断的价值。方法选取88例脑梗塞以及脑出血患者作为观察组,同期选取52例具有神经系统症状的患者作为对照组,采用ELISA法测定血清中8-OHDG表达水平和T-SOD活力,创建ROC曲线,对比单项诊断、平行诊断、系列诊断的灵敏度、特异度等,分析两者在脑梗塞和脑出血诊断中的价值。结果病例组同对照组相比,血清中8-OHDG水平明显增高,T-SOD活力明显降低(P<0.05),8-OHDG、T-SOD诊断的ROC曲线下面积AUC分别为0.7395和0.9247(P<0.001)。在CT排除脑出血后,拟诊断脑梗组鉴别诊断脑梗塞和正常对照组,ROC曲线显示8-OHDG水平、T-SOD活力诊断的曲线下面积AUC分别为0.7115和0.9330(P<0.001)。结论血清8-OHDG水平和T-SOD活力对脑梗塞和脑出血的诊断均有一定价值,联合检测能够提高诊断的灵敏度及特异性。
Objective To evaluate the early diagnostic value of serum 8-OHDG and T-SOD activity in cerebral infarction and cerebral hemorrhage by receiver operator characteristic(ROC) curves. Methods 88 patients with cerebral infarction and cerebral hemorrhage were selected as observation group and 52 patients with nervous system symptoms as control group. Serum 8-OHDG expression and T-SOD activity were measured by ELISA. ROC curve was created. Sensitivity and specificity of serial diagnosis and parallel diagnosis was analyzed. Results Compared with the control group, the serum 8-OHDG level was increased, but T-SOD activity decreased. The areas under the ROC curve for 8-OHDG and T-SOD were 0.7395 and 0.9247, respectively(P<0.001). After intracerebral hemorrhage patients were excluded by CT, the areas under ROC curve for 8-OHDG level and T-SOD level were 0.7115 and 0.9330, respectively(P<0.001). Conclusion The 8-OHDG level and T-SOD activity in serum were useful for diagnosis of cerebral infarction and cerebral hemorrhage, the combination improved the sensitivity and specificity of the diagnosis.
