采用TCT联合HR HPV检测在宫颈癌前病变筛查中的价值

The Value of TCT Combined with HR HPV in the Screening of Cervical Precancerous Lesions

目的探讨采用TCT(液基薄层细胞学)联合HR HPV-DNA(高危型人乳头瘤病毒DNA)检测在宫颈癌前病变筛查中的价值。方法方便选择2017年3月—2018年3月期间来该院就诊的200例宫颈癌前病变筛查妇女为该次研究对象,分别对其进行TCT、HR HPV、TCT联合HR HPV检测及病理活检,以病理活检结果为金标准,比较TCT、HR HPV、TCT联合HR HPV检测的特异性、灵敏性、阴性预测值。结果以病理活检结果为标准,TCT、HR HPV的阳性率分别为50.00%、47.00%,TCT联合HR HPV的阳性率为30.00%,前二者与后者之间的差异有统计学意义(P<0.05);TCT的特异性、敏感性、阴性预测值分别为75.00%、50.00%、75.00%,HR HPV分别为77.32%、53.19%、77.59%,TCT联合HR HPV检测的特异性、敏感性、阴性预测值分别为93.75%、83.33%、93.75%,前二者与后者之间存在的差异有统计学意义(P<0.05),但TCT、HR HPV检测结果的差异无统计学意义(P>0.05),TCT联合HR HPV检测的阳性率、特异性、敏感性、阴性预测值与病理活检结果差异无统计学意义(P>0.05)。结论TCT联合HR HPV检测在宫颈癌前病变筛查中的价值较高,能够提高单一检测的特异性、敏感性、阴性预测值。

Objective To evaluate the value of TCT(liquid based thin layer Cytology)combined with HR HPV-DNA(high risk human papillomavirus DNA)in the screening of cervical precancerous lesions.Methods From March 2017 to March 2018,200 women who came to the hospital to screen cervical precancerous lesions were convenient selected as subjects.TCT,HR HPV,TCT combined with HR HPV and pathological biopsies were performed.The specificity,sensitivity and yin of TCT,HR HPV,TCT combined with HR HPV were compared according to the gold standard of case biopsies.Sexual predictive value.Results According to the results of pathological biopsies,the positive rates of TCT,HR HPV were 50.00%and 47.00%,respectively.The positive rates of TCT combined with HR HPV were 30.00%.There was statistically significant difference between the former two and the latter(P<0.05).The specificity,sensitivity and negative predictive value of TCT were 75.00%,50.00%and 75.00%,respectively.HR HPV was 77.32%,53.19%and 77.59%,respectively.The negative predictive value was 93.75%,83.33%,93.75%.There was statistically significant difference between the first two and the latter(P<0.05),but there was no statistically significant difference in the results of TCT,HR HPV(P>0.05),TCT combined with HR HPV.There was no statistically significant difference in specificity,sensitivity and negative predictive value between them and pathological biopsies(P>0.05).Conclusion TCT combined with HR HPV is of high value in the screening of cervical precancerous lesions,and can improve the specificity,sensitivity and negative predictive value of single detection..
