内镜下支架置入联合择期手术与急诊手术治疗梗阻性结直肠癌远期预后对比分析

Comparative Analysis of Long-term Prognosis of Endobronchial Stenting Combined with Elective Surgery and Emergency Surgery for Obstructive Colorectal Cancer

目的观察急诊手术与内镜下支架置入联合择期手术治疗梗阻性结直肠癌患者远期预后情况并进行分析。方法方便选取2015年1月—2016年1月期间收治的86例结直肠癌患者为对象,随机分为观察组和对照组各43例,对照组采取急诊手术治疗,观察组实施内镜下支架置入联合择期手术治疗,对比两组治疗效果及远期预后情况。结果观察组并发症发生率4.7%、首次排气时间(2.7±1.2)d、住院时间(7.4±0.5)d均少于对照组11.6%、(4.1±2.6)d、(9.8±4.5)d,对比有统计学意义(χ^2=7.980、t=3.450,3.610,P<0.05);两组是否进行辅助化疗、年龄不同、病理组织分期、分级患者预后生存时间对比差异有统计学意义(P<0.05);观察组1年65.1%、3年总生存率44.2%与对照组69.8%、46.5%对比差异无统计学意义(t=0.510、0.610,P>0.05);结论结直肠癌患者采取择期手术联合内镜下支架置入治疗效果显著,值得推广。

Objective To observe and analyze the long-term prognosis of patients with obstructive colorectal cancer treated by emergency surgery and endoscopic stent implantation combined with elective surgery.Methods Convenient elected 86 cases of colorectal cancer patients admitted from January 2015 to January 2016 were selected as the subjects,and were randomly divided into observation group and control group with 43 cases each.The control group was treated with emergency surgery,while the observation group was treated with endoscopic stent implantation combined with elective surgery.The therapeutic effects and long-term prognosis of the two groups were compared.Results The complication rate of observation group was 4.7%,the first exhaust time(2.7±1.2)d,and the hospitalization time(7.4±0.5)d were less than 11.6%,(4.1±2.6)d,(9.8±4.5)d.The comparison was statistically significant(χ^2=7.980,t=3.450,3.610,P<0.05).There was significant difference in the prognosis survival time between the two groups in adjuvant chemotherapy,age,pathological tissue stage,and graded patients(P<0.05).There was no statistically significant difference between the observation group and the 6-year overall survival rate of 44.2%in the observation group and the control group of 69.8%and 46.5%(t=0.510,0.610,P>0.05).Conclusion Selective operation combined with endoscopic stent implantation is effective for colorectal cancer patients and is worthy of promotion.
