比较全身麻醉和椎管内麻醉在全髋关节置换术中的临床应用价值

Comparison of Clinical Application Value of General Anesthesia And Spinal Anesthesia in Total Hip Arthroplasty

目的对比全身麻醉和椎管内麻醉在全髋关节置换术中的临床应用价值。方法方便选择该院全髋关节置换术患者160例,时间设定2017年6月—2018年6月,按照随机数字表法将其分为对照组(全身麻醉,n=80)和观察组(椎管内麻醉,n=80)。对比两组患者的临床效果。结果观察组手术时间和术后麻醉留观时间分别为(92.22±2.78)min、(12.80±1.18)min,均短于对照组(t=13.505,P=0.000,t=13.737,P=0.000);观察组并发症联合发生率、深静脉血栓发生率、术中低血压发生率、输血率发生率及呼吸系统发生率均低于对照组(P<0.05);观察组舒张压、收缩压及心率水平分别为(78.48±6.44)mmHg、(101.22±11.58)mmHg、(66.77±4.48)次/min,均低于对照组﹙t=15.247,P=0.000,t=11.150,P=0.000,t=24.752,P=0.000﹚。结论与全身麻醉相比,椎管内麻醉在全髋关节置换术中麻醉效果更确切,具有较高的临床应用价值。

Objective To compare the clinical value of general anesthesia and spinal anesthesia in total hip arthroplasty.Methods Patients with total hip replacement(160 cases in our hospital)were convenient selected from June 2017 to June 2018.They were divided into control group(general anesthesia,n=80)and observation group(intraspinal anesthesia,n=80)according to random number table method.The clinical effects of the two groups were compared.Results After treatment,the operation time and the observation time of postoperative anesthesia in the observation group were(92.22±2.78)min and(12.80±1.18)min,respectively,shorter than those in the control group(t=13.505,P=0.000,t=13.737,P=0.00);the incidence rates of complications,deep venous thrombosis,intraoperative hypotension,blood transfusion and respiratory system in the observation group were lower than those in the control group(P<0.05);The levels of diastolic blood pressure,systolic blood pressure and heart rate were(78.48±6.44)mmHg,(101.22±11.58)mmHg,(66.77±4.48)times/min,respectively,lower than those of the control group(t=15.247,P=0.000,t=11.150,P=0.000,t=24.752,P=0.000).Conclusion Compared with general anesthesia,intraspinal anesthesia has more accurate anesthetic effect on total hip replacement,and has higher clinical application value.
