单球囊对足月妊娠初产妇促宫颈成熟及引产效果

Effects of Single Balloon on Promoting Cervical Ripening and Induced Labour in Primiparas with Full-term Pregnancy

目的:观察单球囊对足月妊娠初产妇促宫颈成熟及引产效果。方法:选取笔者所在医院2017年1月-2018年12月足月妊娠引产的初产妇160例,按照引产方法分为单球囊组和缩宫素组,单球囊组采取单球囊促宫颈成熟,若无临产可再予人工破膜加缩宫素引产。缩宫素组予缩宫素静滴,必要时联合人工破膜。比较两组临床疗效、产后失血量、总产程时间、治疗前后宫颈Bishop评分、剖宫产率及胎儿窘迫发生率。结果:单球囊组疗效、产后失血量、总产程时间、宫颈Bishop评分均优于缩宫素组(P<0.05)。单球囊组剖宫产率及胎儿窘迫发生率均低于缩宫素组(P<0.05)。结论:单球囊对于足月妊娠初产妇促宫颈成熟及引产的效果确切,可有效缩短产程时间,减少产后失血,降低剖宫产率及胎儿窘迫发生率。

Objective:To observe the effects of single balloon on promoting cervical ripening and induced labor in primiparas with full-term pregnancy.Method:A total of 160 primiparas with full-term pregnancy who were induced labor from January 2017 to December 2018 in our hospital were enrolled.Primiparas were divided into the single balloon group and the Oxytocin group according to the induced labor method.The single balloon group used a single balloon to promote cervical ripening.If there is no labor,induced labor of artificial rupture of membrane and Oxytocin can be given.The Oxytocin group was given intravenous Oxytocin,and if necessary,combined with artificial rupture of membrane.The clinical efficacy,postpartum blood loss,total labor time,cervical Bishop score before and after treatment,rate of cesarean section and incidence of fetal distress were compared between the two groups.Result:The efficacy,postpartum blood loss,total labor time and cervical Bishop score in the single balloon group were better than those of the Oxytocin group(P<0.05).The rate of cesarean section and incidence of fetal distress in the single balloon group were lower than those of the Oxytocin group(P<0.05).Conclusion:Single balloon has a definite effect of promoting cervical ripening and induced labor in primiparas with full-term pregnancy,which can effectively shorten the time of labor,reduce postpartum blood loss,reduce the rate of cesarean section and the incidence of fetal distress.
