高血压患者P波离散度与动态血压参数的相关性

Correlation between P Wave Dispersion and Dynamic Blood Pressure Parameters in Patients with Hypertension

目的:分析高血压患者P波离散度与动态血压参数的相关性。方法:将2017年12月-2018年11月笔者所在医院收治的176例高血压患者作为观察组,选择同一时期于笔者所在医院正常体检的94例健康者为对照组,所有患者均进行24 h动态血压监测及12导联心电图检查。对比两组P波最大时限(Pmax)、P波最小时限(Pmin)及P波离散度(Pd),对动态血压参数和Pd相关性进行分析。结果:观察组Pmax和Pd值均高于对照组,差异均有统计学意义(P<0.05);两组Pmin值比较,差异无统计学意义(P>0.05)。经Pearson分析显示,Pd与24 h平均收缩压、收缩压负荷、白天平均收缩压、夜间平均收缩压均为正相关(P<0.05)。结论:高血压患者P波离散度与收缩压及收缩压负荷具有密切关系,临床可通过上述指标对患者的心房损害程度进行评估。

Objective:To analyze the correlation between P wave dispersion and dynamic blood pressure parameters in patients with hypertension.Method:A total of 176 patients with hypertension admitted to our hospital from December 2017 to November 2018 were selected as the observation group,and 94 healthy people who underwent the normal physical examination in our hospital during the same period were selected as the control group.All patients were tested for 24 h dynamic blood pressure and 12-lead ECG.P wave maximum time limit(Pmax),P wave minimum time limit(Pmin)and P wave dispersion(Pd)were compared between the two groups to analyze the correlation between dynamic blood pressure parameters and Pd.Result:Pmax and Pd in the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).Pmin were compared between the two groups,and the difference was statistically significant(P>0.05).Pearson analysis showed that Pd was positively correlated with 24 h mean systolic blood pressure,systolic blood pressure load,daytime mean systolic blood pressure,and nighttime mean systolic blood pressure(P<0.05).Conclusion:P wave dispersion in patients with hypertension is closely related to systolic blood pressure and systolic blood pressure load.The degree of atrial damage can be evaluated by the above indicators.
