拉玛泽呼吸法联合家属全程陪伴在瘢痕子宫再妊娠经阴道分娩护理中的应用

Application of Lamaze Respiration Method and Whole Course Accompaniment of Family Members in the Nursing of Transvaginal Delivery for Scar Uterine Repregnancy

目的:探讨拉玛泽呼吸法联合家属全程陪伴在瘢痕子宫再妊娠经阴道分娩护理中的应用。方法:回顾性分析2017年3月-2019年3月笔者所在医院收治的84例瘢痕子宫再妊娠经阴道分娩孕妇的临床资料,按照护理方式的不同分为对照组和观察组,各42例。其中对照组给予常规护理进行干预,观察组给予拉玛泽呼吸法联合家属全程陪伴进行干预。比较两组负性情绪、阴道分娩成功率、分娩结局及经阴道分娩孕妇的产程情况。结果:观察组干预后SDS和SAS评分均低于对照组,差异均有统计学意义(P<0.05)。观察组阴道分娩成功率为95.23%,高于对照组的73.81%,差异有统计学意义(P<0.05)。观察组不良妊娠率为7.14%,低于对照组的26.19%,差异有统计学意义(P<0.05)。观察组成功经阴道分娩孕妇第一、二、三及总产程时间均短于对照组,差异均有统计学意义(P<0.05)。结论:在瘢痕子宫再妊娠经阴道分娩护理中应用拉玛泽呼吸法联合家属全程陪伴能够明显改善孕妇的负性情绪,提高阴道分娩成功率,降低不良妊娠率,缩短产程时间,值得在临床上大力推广。

Objective:To explore the application of Lamaze breathing method combined with family accompanying in the nursing of vaginal delivery of scarred uterus pregnancy.Method:A retrospective analysis was made of the clinical data of 84 pregnant women with scarred uterus who underwent vaginal delivery from March 2017 to March 2019.They were divided into the control group and the observation group according to different nursing methods,42 cases in each group.The control group was given routine nursing intervention,while the observation group was given Lamaze breathing method combined with accompanying family members throughout the intervention.The negative emotions,the success rate of vaginal delivery,the outcome of delivery and the delivery process of the two groups were compared and analyzed.Result:The scores of SDS and SAS in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The success rate of vaginal delivery in the observation group was 95.23%,which was higher than 73.81%in the control group,the difference was statistically significant(P<0.05).The adverse pregnancy rate of pregnant women in observation group was 7.14%,which was lower than 26.19%in control group,the difference was statistically significant(P<0.05).The first,second,third and total stages of successful transvaginal delivery of pregnant women in the observation group were shorter than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The application of Lamaze breathing method combined with family accompanying during the whole process of vaginal delivery nursing for scarred uterus pregnancy can significantly improve the negative mood of pregnant women,improve the success rate of vaginal delivery,reduce the rate of adverse pregnancy and shorten the duration of labor,which is worthy of vigorous promotion in clinical practice.
