针刺联合穴位埋线治疗坐骨神经痛的效果

Effect of acupunctural therapy combined with acupoint catgut embedding in the treatment of sciatica

目的比较针刺联合穴位埋线与常规针刺治疗坐骨神经痛的效果差异。方法选取2016年1月~2017年9月弥勒市中医医院针灸推拿科门诊及住院部收治的72例坐骨神经痛患者作为研究对象,采用随机数字表法将其分为针刺组和埋线组,每组各36例。针刺组患者采用常规针刺治疗,埋线组患者采用普通针刺加穴位埋线治疗。比较两组患者的治疗总有效率。比较两组患者治疗5、10 d的疼痛视觉模拟量表(VAS)评分、六点行为(BRS-6)评分、改良日本骨科学会下腰痛(JOA)评分及结束治疗3个月后随访的JOA评分。结果治疗10 d,埋线组患者的治疗总有效率(97.22%)高于针刺组(86.11%),差异有统计学意义(P<0.05)。治疗5、10 d,埋线组患者的VAS、BRS-6评分低于针刺组,差异有统计学意义(P<0.05)。治疗5、10 d,埋线组患者的JOA评分高于针刺组,差异有统计学意义(P<0.05)。治疗结束3个月随访,埋线组患者的治疗总改善率(100.00%)高于针刺组(94.44%),差异有统计学意义(P<0.05)。结论针刺联合穴位埋线治疗坐骨神经痛,不仅具有明显的短时镇痛效应,而且远期疗效显著,复发率低,值得临床推广应用。

Objective To compare the effect of acupunctural therapy combined with acupoint catgut embedding and conventional acupunctural in the treatment of sciatica.Methods From January 2016 to September 30,2017,72 patients with sciatica from the department of acupuncture and massage in both outpatient clinic and inpatient of Traditional Chinese Medicine Hospital of Mile City were selected as the study subjects.They were divided into acupuncture group and embedding group by the random number table method,with 36 cases in each group.Patients in the acupuncture group were treated with conventional acupunctural therapy,and patients in the embedding group were treated with acupunctural therapy combined with acupoint catgut embedding.The total therapeutic efficiency of the two groups were compared.The visual analogue scale(VAS)score on pain at 5 and 10 days after treatment,the six-point behavioral rating(BRS-6)score,the modified Japanese Orthopedics Association lower back pain(JOA)score,and the follow-up JOA score at 3 months after treatment were compared between the two groups.Results After 10 days of treatment,the total effectiveness rate of the embedding group was 97.22%,which was higher than that of the acupuncture group accounting for 86.11%,with statistically significant difference(P<0.05).After 5 and 10 days of treatment,the VAS and BRS-6 scores in the embedding group were lower than those in the acupuncture group,the differences were statistically significant(P<0.05).The JOA score in the embedding group was higher than that in the acupuncture group,with statistically significant difference(P<0.05).After 3 months of follow-up,the total improvement rate(100.00%)of patients in the embedding group was higher than that in the acupuncture group(94.44%),with statistically significant difference(P<0.05).Conclusion Acupunctural therapy combined with acupoint catgut embedding in the treatment of patients with sciatica not only obtains an obvious short-term analgesic effect,but also has a long-term curative effect with a low recurrence rate.It is worthy of clinical application.
