椎间孔镜术后副损伤导致腰臀部疼痛原因分析

Analysis of the causes of pain in the lower back and hip caused by the secondary injury after intervertebral foramen

目的:分析椎间孔镜术后副损伤导致腰臀部疼痛的原因。方法:2018年1月-2019年1月选取经椎间孔内镜下髓核摘除术患者60例,在术后1、6个月对患者进行腰椎稳定性评定,同时结合患者自身意愿在复查过程中是否接受逐层阻滞分为阻滞组与非阻滞组,分析患者不同时间段腰痛ODI、VAS评分。结果:60例患者均在术后1个月内进行复查,无腰椎不稳现象。所有患者手术前1 d与术后6个月腰痛ODI、VAS评分比较,差异有统计学意义(P<0.05)。结论:椎间孔镜术后副损伤导致腰臀部疼痛的原因主要是以纤维环损伤、椎小关节囊损伤为主,而腰臀部疼痛程度并不会对腰椎功能恢复造成影响。

Objective:To analyze the causes of pain in the lower back and hip caused by the secondary injury after intervertebral foramen.Methods:60 patients with endoscopic nucleus pulposus removal were selected from January 2018 to January 2019,the lumbar stability was assessed at 1 month and 6 months after surgery,at the same time,according to the patient's own wishes,whether to accept the layer-by-layer block in the review process,they were divided into the block group and the non-block group.We analyzed ODI and VAS scores of patients with low back pain in different time periods.Results:60 patients were reexamined within 1 month after operation,and there was no lumbar instability.There was significant difference in ODI and VAS scores of lumbago between 1 day before operation and 6 months after operation(P<0.05).Conclusion:The causes of lumbar and buttock pain caused by secondary injury after intervertebral foramen are mainly caused by annulus fibrosus injury and vertebral facet joint capsule injury,and the degree of pain in the waist and buttocks will not affect the recovery of lumbar function.
