精神分裂症患者甲状腺激素变化特征及其与白蛋白的相关性

Characteristics of thyroid hormone changes and their correlation with albumin in patients with schizophrenia

目的探讨精神分裂症患者血清甲状腺激素的变化特征及其与白蛋白(ALB)的相关性。方法选取西安市精神卫生中心门诊接收的精神分裂症患者80例为疾病组,同时选取西安市友谊医院的健康体检者46名为对照组。采用化学发光法检测两组的血清促甲状腺激素(TSH)、三碘甲状腺原氨酸(T 3)、甲状腺素(T 4)、游离三碘甲状腺原氨酸(FT 3)、游离甲状腺素(FT 4)、反三碘甲状腺原氨酸(rT 3)的水平,同时采用溴甲酚紫终点法检测疾病组患者的ALB水平。结果疾病组的TSH、FT 4、rT 3水平均高于对照组,T 3、FT 3水平均低于对照组,差异有统计学意义(P<0.05),两组间T 4比较,差异无统计学意义(P>0.05)。病程和性别对疾病组患者甲状腺激素指标(TSH,T 3,T 4,FT 3,FT 4,rT 3,T 3/rT 3)的影响差异均无统计学意义(均P>0.05);疾病组患者T 3/rT 3比值与ALB水平呈负相关(r=-0.262,P=0.019)。结论精神分裂症患者存在甲状腺功能紊乱,rT 3升高是其特征性变化,T 3/rT 3比值与ALB水平呈负相关。

Objective To investigate the change characteristics of serum thyroid hormone and its correlation with albu-min(ALB)in patients with schizophrenia.Methods Eighty patients with schizophrenia admitted by the outpatient de-partment of Xi′an Mental Health Center were selected as the disease group,and forty-six healthy patients from Xi′an Friendship Hospital were selected as the control group.Serum levels of thyrotrophin(TSH),triiodinated thyroxine(T 3),thyroxine(T 4),free triiodinated thyroxine(FT 3),free thyroxine(FT 4),and reverse triiodothyronine(rT 3)in two groups were detected by chemical radiometry.At the same time,the ALB level of patients in the disease group was detected by the end point method of bromocresol purple.Results TSH,FT 4 and rT 3 levels in the disease group were all higher than those in the control group,while T 3 and FT 3 levels were lower than those in the control group,with statistically significant differences(P<0.05).There was no significant difference in T 4 between the two groups(P>0.05).There were no statistically significant differences in the effects of disease course and gender on thyroid hormone indexes(TSH,T 3,T 4,FT 3,FT 4,rT 3,T 3/rT 3)in the disease group(all P>0.05).In the disease group,the T 3/rT 3 ratio was nega-tively correlated with ALB level(r=-0.262,P=0.019).Conclusion In schizophrenia patients with thyroid dysfunction,rT 3 elevation is the characteris-tic change,and the T 3/rT 3 ratio is negatively corre-lated with ALB level.
