双侧完全性唇裂术后继发鼻唇畸形特点研究

Related Research of the Characteristics in Bilateral Complete Cleft Lip Postoperative Nasolabial Deformity

目的探讨双侧完全性唇裂术后继发鼻唇畸形的特征。方法收集由2011年1月2015年1月宁夏医科大学总医院行完全性双侧唇裂修复术并同期行鼻畸形开放矫正术的患儿为病例组(病例1组为术后1~3年;病例2组为术后3~5年),正常同年龄段的儿童为对照组(对照1组为1~3岁;对照2组为3~6岁)。采用直线数据(12项)与集角度数据(4项)双向共同分析,得出其鼻唇体表解剖特点。结果病例1组与对照1组测量项目比较结果显示,两组间唇宽比较差异无统计学意义,其余15项测量项目比较差异均有统计学意义(P均<0.05)。病例2组与对照2组测量项目比较结果显示,除唇宽、鼻翼长度、侧唇厚度、唇珠厚度比较差异无统计学意义外,其余测量项目比较差异均有统计学意义(P均<0.05)。结论双侧完全性唇裂患儿术后鼻唇部各观测项目与正常同龄儿童有差异,为临床治疗提供理论依据。

Objective To explore the characteristics of secondary nasolabial deformity after bilateral complete cleft lip operation. Methods From January 2011 to January 2015,the children who had complete bilateral cleft lip repair and open rhinoplasty at the same time in our hospital were selected as case group(case group 1 was l-3 years;case group 2 was 3-5 years). Normal children of the same age were taken as the control group(control group 1 was 1-3 years old;control group 2 was 3-6 years). The anatomic characteristics of nasolabial body surface were obtained by two-way analysis of line data(12 items)and angle data(4 items). Results After comparison of the measured values in the patient after surgery 1-3 years,it was found that except the lip(P>0.05),the other 15 test indicators(P<0.05)indicating significant difference. After comparison of the measured values in the patients after surgery more than 3 years and less than 5 years,it was found that there was no significant difference in lip width still,alar length,lateral lip thickness,(P>0.05)so there was no significant difference. All the other items(P<0.05),indicating significant differences. According to the measured value comparison of the bilateral symmetry project of the nose and lips of the children in each group(P>0.05)indicating no significant difference. Conclusion In children with bilateral complete cleft lip,most of the observation items on the nasal lips are significantly different from that of normal children of the same age.
