显微血管技术结合穿支皮瓣修复手外伤创面的临床效果

Clinical Effect of Microvascular Technique Combined with Perforator Flap in Repairing Hand Wound

目的探究显微血管技术结合穿支皮瓣修复手外伤创面的临床效果。方法选取我院2013年7月-2018年6月收治的45例手外伤软组织缺损患者作为研究对象,均实施显微血管技术结合穿支皮瓣修复术治疗,分析临床治疗效果、症状评分及白细胞介素-6(IL-6)、白细胞介素-8(IL-8)水平、并发症发生率。结果全部患者接受治疗后,皮瓣均成活。质地好,外形可,恢复部分感觉,供区愈合可,优良率为95.56%。在VAS评分、瘙痒评分、症状表现和IL-6、IL-8水平炎症因子水平方面,治疗后均低于治疗前(P均<0.05)。随访2~12个月,平均6个月,发生皮瓣感染1例,皮瓣肿胀1例,皮瓣液化1例,并发症发生率占6.67%,经相应的处理后,均消失。结论穿支皮瓣修复术治疗手外伤软组织缺损患者可改善患者的临床症状或表现,抑制相关炎症因子的表达,安全性较高,值得推广应用。

Objective To explore the clinical effect of repairing hand trauma with microvascular technique combined with perforator flap.Methods 45 cases of hand trauma soft tissue defects admitted to our hospital from July 2013 to June 2018 were selected as research objects,all patients were treated with microvascular technique combined with perforator flap,Analysis of the clinical curative effect,symptom scores and inflammatory factor levels and incidence of complications.Results All the flaps survived without necrosis.Good texture,shape,recovery of part of the feeling,for the healing area,good rate of 95.56%.In terms of VAS score,pruritus score,IL-6 level and IL-8 level,inflammatory cytokines were better after treatment than before treatment,and the difference was statistically significant(P all<0.05).The follow-up time was 2 to12 months,with an average of 6 months.There was 1 case of skin flap infection,1 case of skin flap swelling,and 1 case of skin flap liquefaction.The complication rate was 6.67%.Conclusion Perforator flap was used to repair the soft tissue defect of hand injury,it can improve the clinical symptoms or manifestations of patients,inhibit the expression of related inflammatory factors,and is safe and worthy of promotion and application.
