中性粒细胞功能在慢性阻塞性肺疾病患者中的变化及临床意义

The changes of neutrophil function and clinical significance in chronic obstructive pulmonary disease patients

目的:探讨慢性阻塞性肺疾病(简称慢阻肺)患者急性加重期及稳定期的中性粒细胞功能变化及临床意义。方法:选取60例慢阻肺急性加重期患者和60例稳定期患者,测定患者血浆中性粒细胞明胶酶(neutrophil gelatinase-associated lipocalin,NGAL)和弹性蛋白(neutrophil elastase,NE)水平,并进行相关分析。结果:急性加重期组NGAL及NE水平比稳定期明显升高,分别为(120.46±26.52)μg/L vs(80.22±16.24)μg/L和(5.78±0.52)nmol/L vs(3.24±0.61)nmol/L,差异有统计学意义(P<0.05);急性加重期慢阻肺患者N GAL和NE在肺功能轻至中度的相关系数分别为-0.425、-0.448,在肺功能为重至极重度的相关系数分别为-0.636、-0.724;稳定期慢阻肺患者NGAL和NE在肺功能轻至中度的相关系数分别为-0.124、-0.214,在肺功能为重至极重度的相关系数分别为-0.226、-0.221。结论:NGAL和NE可反应慢阻肺的严重程度,可作为慢阻肺急性加重期的血清标志物,可用于评估病情及疗效。

Objective To investigate the changes of neutrophil function and its clinical significance in chronic obstructive pulmonary disease(COPD)patients at acute exacerbation stage and stable stage.Method The study involved 60 patients with exacerbated COPD and 60 patients with stable COPD.Plasma neutrophil gelatinase-associated lipocalin(NGAL)and elastic protein elastase(NE)level were measured and compared between two groups and related analysis.Results The levels of NGAL and NE in the acute exacerbation group were significantly higher than those in the stable group,(120.46±26.52)μg/L vs(80.22±16.24)μg/L and(5.78±0.52)nmol/L vs(3.24±0.61)nmol/L,respectively(P<0.05);The correlation coefficients of NGAL and NE in patients with acute exacerbation of COPD were-0.425 and-0.448,respectively,the correlation coefficient of lung function,and very severe weight were-0.636 and-0.724,respectively;stable COPD patients with NGAL and NE in lung function in mild to moderate the correlation coefficient were-0.124,-0.214,respectively,lung function in the correlation coefficient of heavy and very severe were-0.226 and-0.221,respectively.Conclusion NGAL and NE can reflect the severity of COPD,and can be used as a serum marker for the acute exacerbation of COPD,and can be used to assess the disease and the curative effect.
