不同剂型的芪丹通络汤治疗糖尿病肾病Ⅲ期临床随机对照研究

Study on Effect of Different Types of Qidan Tongluo Decoction in Treatment of Diabetic Nephropathy of Stage 3

目的:探讨比较芪丹通络汤颗粒制剂与传统水煎剂治疗3期糖尿病肾病的临床疗效。方法:选取医院收治的3期糖尿病肾病患者158例,按随机数字表法分组,对照组79例予以芪丹通络汤剂治疗,研究组79例予以芪丹通络颗粒治疗,比较两组间肾功能、血液流变学指标水平、氧化应激指标水平,以及临床疗效。结果:对照组治疗有效率(68.36%)明显低于研究组(87.34%),具有统计学意义(P<0.05);与对照组比较,研究组治疗后血清肌酐(Scr)、尿素氮(BUN)水平及24 h尿白蛋白定量(UAE)水平较高,治疗后全血高切黏度、全血低切黏度、血浆黏度及血小板聚集率较低,治疗后血清脂联素(ADPN)、丙二醛(MDA)水平较低,超氧化物歧化酶(SOD)水平较高,具有统计学意义(P<0.05)。结论:芪丹通络颗粒剂治疗3期糖尿病肾病的临床疗效较好,能明显改善肾功能,减少尿蛋白排泄,缓解血液高黏状态,提高机体抗氧化能力,抑制氧化应激反应,延缓病情发展,治疗作用优于传统中药汤剂。

Objective:To investigate the effect of different types of Qidan Tongluo Decoction in the treatment of diabetic nephropathy of stage 3.Methods:A total of 158 patients with 3 stage diabetic nephropathy in our hospital were selected and divided into two groups.Seventy-nine cases in the control group treated with self-made Qidan Tongluo Decoction and 79 cases in the experiment group treated with Qidan Tongluo Granules.The indexes of renal function,blood rheology and the level of oxidative stress and the clinical efficacy were compared.Results:The effective rate in the control group(68.36%)was lower than that in the experiment group(87.34%),and the difference was statistically significant(P<0.05).Compared with the control group,serum levels of creatinine(Scr),urea nitrogen(BUN)and 24 h urinary albumin quantification(UAE)levels were higher after treatment in the experiment group.The whole blood high viscosity,whole blood low shear viscosity,plasma viscosity and platelet aggregation rate were lower after treatment.The serum adiponectin(ADPN)and malondialdehyde(MDA)levels were lower,and superoxide dismutase(SOD)levels were higher after treatment with significant difference(P<0.05).Conclusion:The efficacy of Qidan Tongluo Granules in the treatment of 3 stage diabetic nephropathy is high.It can obviously improve renal function,reduce urinary protein excretion,and can relieve the high viscosity of blood,improve the antioxidant capacity of the body,inhibit oxidative stress reaction,and delay the development of the disease.The therapeutic effect is better than that of the traditional herbs decoction.
