B超引导下选择性神经根阻滞用于肩关节术后镇痛及对肩关节功能的影响 被引量:2

摘要 目的观察B超引导下选择性神经根阻滞用于肩关节术后镇痛及对肩关节功能的影响。方法 2014年9月-2018年8月选择在我院进行冻结肩肩关节镜手术患者110例,根据术后镇痛方法的不同分为观察组56例与对照组54例。对照组给予经静脉通路镇痛,观察组给予B超引导下选择性神经根阻滞,记录2组镇痛效果与预后疗效。结果 2组术后4 h与术后12 h的疼痛VAS评分都显著低于术前1 d(P<0.05),观察组疼痛VAS评分也显著低于对照组(P<0.05)。2组术后7 d与术后14 d的疼痛肩关节功能评分都显著高于术前1 d(P<0.05),观察组肩关节功能评分也显著高于对照组(P<0.05)。结论 B超引导下选择性神经根阻滞应用在肩关节术后镇痛能提高镇痛效果,改善患者的肩关节功能,值得临床推广应用。 Objective To observe the effect of selective nerve root block guided by B-ultrasound in postoperative analgesia of shoulder joint. Methods From September 2014 to August 2018, 110 patients with frozen shoulder and shoulder arthroscopy were selected in our hospital. According to the different postoperative analgesia methods, 56 patients in the observation group and 54 patients in the control group. After analgesia via venous access, the observation group was given selective nerve root block guided by B-ultrasound, and the analgesic effect and prognosis of the two groups were recorded. Results Both groups had significantly lower VAS scores 4h after surgery and 12h after surgery than 1d before surgery (P<0.05), and the VAS scores in the observation group were also significantly lower than those in the control group (P<0.05). Both groups had significantly higher scores of shoulder joint function on postoperative day 7 and postoperative day 14 (P<0.05), and the scores of shoulder joint function in the observation group were also significantly higher than those in the control group (P<0.05). Conclusion The application of selective nerve root block under the guidance of B ultrasound in postoperative analgesia of shoulder joint can improve the analgesic effect and improve the shoulder joint function of patients, which is worthy of clinical application.
