骨粉联合富血小板纤维蛋白治疗慢性牙周炎患者骨缺损临床观察

Clinical Observation of Bone Powder Combined with PRF in the Treatment of Bone Defects in Patients with Chronic Periodontitis

目的:探讨富血小板纤维蛋白(PRF)和Bio-Oss骨粉联合应用治疗慢性牙周炎患者骨缺损的临床疗效。方法:慢性牙周炎患者50例,均因并发骨缺损拟行植骨术治疗,随机分为两组各25例,A组单纯使用Bio-Oss骨粉修补骨缺损,B组使用PRF联合Bio-Oss骨粉修补骨缺损。比较两组患者术前与术后3个月探诊深度(PD)、附着丧失(AL)、Mazza出血指数(BI)等相关指标变化,以及创面愈合情况和患者满意比例。结果:术后两组患者PD、AL、BI等指标均明显优于术前(P<0.05),且B组患者术后的PD、AL,以及创面愈合情况、患者满意比例等均明显优于A组(P<0.05),两组患者围术期不良事件发生率差异无统计学意义(P>0.05),X线检查显示两种治疗方案均效果确切(P>0.05)。结论:PRF联合Bio-Oss骨粉治疗慢性牙周炎源性骨缺损,在修补骨缺损、促进骨生长、骨愈合方面比单独应用骨粉效果更好,值得临床推广。

Objective:To explore the clinical efficacy of the combined application of platelet-rich fibrin(PRF)and Bio-Oss bone powder in the treatment of bone defects.Methods:Totally 50 cases of clinically confirmed chronic periodontitis patients admitted to the department of stomatology of our hospital from January 2016 to January 2018 were collected.All of them were treated with bone grafting due to bone defect.The patients were randomly divided into 2 groups:25 cases were treated with Bio-Oss bone powder to repair bone defects(group A),and another 25 cases were treated with PRF combined with Bio-Oss bone powder to repair bone defects(group B).The clinical indicators,such as probing depth(PD),loss of attachment(AL),Mazza bleeding index(BI),wound healing and patient satisfaction were compared before and after surgery.Results:PD,AL,BI and the other indicators of the 2 groups of patients after surgery were significantly better than those before surgery(P<0.05),and PD,AL,wound healing and patient satisfaction in group B three months after surgery were significantly better than those in group A(P<0.05).There was no statistically significant difference in the incidence of perioperative adverse events between the groups(P>0.05),and X-ray examination showed that the two treatment regiments were both effective(P>0.05).Conclusion:The combination of PRF and Bio-Oss bone powder in the treatment of chronic periodontitis bone defects is better than the single application of bone powder in repairing bone defects,and promoting bone growth and bone healing,which is worthy of clinical promotion.
