动脉血乳酸联合血糖监测在新生儿窒息诊断中的意义

目的 探讨动脉血乳酸联合血糖监测在新生儿窒息诊断中的临床价值.方法 随机选取2017年1月至12月新生儿科收治的104例发生窒息的足月新生儿为观察组,依据Apgar评分分为轻、重窒息两组,并随机选取同期无围生期缺氧及感染的足月新生儿46例设为对照组,统计分析动脉血乳酸和血糖水平与新生儿窒息的关系.结果 观察组血乳酸和血糖水平显著高于对照组,差异有统计学意义(P<0.05),血乳酸和血糖水平越高,其并发症发生率明显增高,差异均有统计学意义(P<0.01).结论 动脉血乳酸联合血糖监测可作为临床判断新生儿窒息程度的辅助实验室指标,对指导治疗和预测预后具有重要的意义.

Objective To investigate the clinical value of the monitoring of arterial blood lactic acid combined with blood sugar levels in the diagnosis of neonatal asphyxia.Methods 104 full—tenn newborns with asphyxia in our department of neonatology,the maternal and child health care center of huzhou from January 2017 to December 2017 were randomly selected as an observation group.According to the Apgar score they were divided into 2 groups:light asphyxia group and heavy asphyxia group.And meanwhile 46 full—term newborns without perinatal hypoxia and infection were randomly selected as a control group.The relationship between neonatal asphyxia and both arterial blood lactic acid and blood sugar levels were analyzed.Results Blood lactic acid and blood sugar levels in the observation group were significantly higher than those in the control group,the difference was statistically significant(P<0.05).The higher the level of blood lactate and blood glucose,the higher the incidence of complications,and the difference was statistically significant(P<0.01).Conclusion The monitoring of arterial blood lactic acid combined with blood sugar levek can be used as an auxiliary laboratory index of clinical judgment of neonatal asphyxia,and it has very important significance to guide the treatment and in predicring the prognosis.
